Conflicting info about heart disease.

I have some in my family history, so have been reading recently.
What I have noticed is that the science seems to be all over the place. Or else the reporting of the science is all over the place... as not sure we can blame the scientists for this.

When researching, most sources claim that Coronary Heart Disease is highest in countries that are wealthier, ie, the west. There is a good amount of info that correlates rich countries with higher rates of heart disease, with Korea, Japan and France (not poor), at the lowest.

However, the very highest rates are to be found in the middle east, north Africa and Russia. The actual results do not tally at all with the wealthiest countries, and are astonishingly problematic in areas such as Afghanistan, Ukraine, etc. (world map of heart disease rates here).

This is a pretty conflicting bunch of information, and doesn't help to make sense of it as a global issue at all.
Is anyone else baffled by this, or know why the reported stats are so muddled?
