Difficult work decision

I’m a few months in to a new job, that’s completely different to my previous career. There have been a few performance issues but swiftly rectified and at my mid way probation period boy positive comments. Since then I’ve had to take compassionate leave and I’m really struggling with my mental health, I’m having private psychiatric consultant appointments and he thinks I should be signed off.

I kind of feel like if I do that I might as well resign, it’s not great being off for stress while still in probation period but also if try and work I’m going to be distracted, probably make mistakes and unreliable. I do enjoy my job, the company is great and it’s the best manager I’ve ever had. It’s just bad timing.

I’m sort of thinking when I go back tomorrow after my leave I have a chat with my manager and explain and acknowledge the time and expense hiring and training me and I’m happy to try and continue but I’m not the version of me they hired. Or I offer to resign to save the awkwardness of them letting me go? What would you do?

we are only a very small team, with one of them already off on long term sick and coming in to our busiest time of the year. Neither solution is great.
