Go to your local garage and they will help you.

Although do your sums. Motability isnt always the cheapest way to obtain a car.

Look for garages that offer mobility cars ,it's pretty straightforward but you need to be aware that ,that depending on what car you want there can sometimes be a quire hefty deposit .

If your the driver than your driving licence and proof you get HRM ,if someone else going to be the driver, they would need to come with you with their driving licence.

I have a motability car and I love it. I got a car (Qashqai) that can store my wheelchair and is easy for me to get in and out of. Have a look at the motability site and get an idea of what sort of car you'd like, but most garages provide them now. You need proof of eligibility. I found it really easy - although I had to wait 5 months for my car. I can't just walk round the corner to the shops, or walk across a few streets to see my friends so it's been invaluable for me.

Motabilty have a website that should explain it all, including participating garages.

Be careful, and don't make immediate decisions until you understand all the pros and cons. . Motability scheme is good and works well for many, especially if you're not working dependent, but not for all.

I need a WAV and I'm self employed. Any issues with garage timetables, service availability, breakdowns, DWP, and my income and reliability stops dead.

It turned out to be cheaper, more reliable, and importantly for me, massively more independent (if assessors suddenly award nil points later and you have to go through mandatory reconsideration etc) to take a loan, and buy and adapt an older vehicle that held it's value well, and use the pip to pay for it. If anything goes wrong, I still have a saleable vehicle.
Just a thought.

Good luck whatever you do, hoping freedom beckons. 🙂

As mentioned, the Motability website should have the information you need to get started. You can identify a car that you might like there, including the deposit required and so on and the relevant local dealer should hold your hand through the application process.

The deposit prices change every quarter but if you apply in June and then the downpayment price for that vehicle changes in July you still pay the June price, whether it's more or less. (Yes, we got stung after ordering in December!)
