urgent gynae advise needed for teenager please

life's been a bit chaotic and I just can't think straight, so this might be obvious

dd13 started with what we thought was severe menstrual cramps on Saturday. this is normal for her but this was more extreme.... vomiting and fainted. but me and other dd experience bad periods so horrid but didn't worry too much

it has improved since but she is still getting waves of pain where she can't walk, peeing hurts, keeping her awake etc etc

it occurred to us that she has not long finished a period and so we went to OOH last night. they ruled out appendicitis and urine infection, and do think it's gynae. they didn't have access to a CT scanner and told us to see GP today.

I have been trying to contact GP anyway since yesterday and have received a text message saying phone lines are down, should get a response in 2 days

I think she needs a scan. I don't think she should wait 6 months for GP to refer her to gynae. but currently she isn't in extreme pain so I don't feel like we can go to AnE. she fell asleep about 6am so is currently settled

Could it be a cyst? Definitely sounds like she needs a scan. A&E might be your only option, as you say referrals take forever these days.

Could be kidney stones; usually presents with waves of pain, can make it painful to pass urine, vomiting, etc. Another sign would be microscopic heamaturia (on a urine dipstick) but you said she’d not long finished a period, so has she actually had any vaginal bleeding during this episode of pain? - because that would obviously show up on a dipstick and clouds the picture about whether it’s vaginal blood showing on the dipstick or blood from the kidneys/ureters/bladder.

I’d see how she is when she wakes up, if still having waves of severe pain take her to A&E, but if she seems to have improved then own GP.

dipstick was done last night by nurse and urine completely clear. no, no vaginal bleeding since Saturday and isn't due a period. I wondered if it might be related to ovulation

grrrr....a text from GP has just come through. sending prescription for tranexamic acid. and we will receive a routine appointment to discuss scan (so that will be 3 weeks plus) but reassures us that usually no structural abnormalities at this age

If my 13 year old DD was in so much pain that she vomited and passed out it wouldn't occur to me NOT to go to A&E - and between three children I can count the A&E visits we've made on the finger of one hand. I hope you get some answers and she feels better soon.

Gynae waits are long. I've been on morphine for my period pain and unable to work for 1-2 days a month so I'm going to sound harsh but they can't just push her to the top of the list
12 months is standard where I am and that's after an ultrasound that shows I have endometriosis

May be Endo but that doesnt show on a scan.
DD started her periods age 12 and the first few were ok, which I was very relieved about as we have a family history of severe periods.
I am sorry to say OP that it was the start of a very long journey involving a lot of useless GP's who gave pointless advice such as meditation and exercise or a hot water bottle. As DD said "once I stop throwing up and can actually get up off the bathroom floor I could try a bit of colouring inbetween fainting"
Eventually I paid for a Private Gynae who suggested a certain medication but the GP was still resistent until the Gynae phoned up and I am not sure what she said but it was prescribed next day.
I really hope that this is a one off for your DD or if not she has more luck than we did getting help.

I was reading about Pelvic Inflammatory disease....does anyone here have experience of that?

I've had similar it was a cyst and ovarian torsion. If pain hadn't gone I'd go to an and e, gynae problems are too often overlooked.

As a GP has been suggested, I'd go there now and try and get an appointment for today. If they won't see her today and she's definitely still in pain, then go to A&E.

Go to a and e. Your DD needs medical assistance. Pack a bag with drinks, etc and go.

yeah, I get that. but before hysterectomy that was normal for me when I had a period....for about 30 years. and my other daughter was the same, but we went and got her a coil which fixed it. so, I appreciate that I underreact to period pains. I had endo and adenomyosis and chronic anaemia

it's not about pushing her to the top of lists for a chronic gynae problem. it's about trying to identify if she has something that needs seeing urgently. there is a big vacuum isn't there for things that aren't a now now life threatening emergency and things that can wait 6 months for a routine appointment. if it is PID, it could make her infertile for eg

Bloody hell that sounds awful, so sorry you had to go through that. I reread my post and realised it might have sounded like I was questioning why you hadn't just taken her already, sorry I didn't mean it that way, I just meant that you shouldn't feel like you are overreacting by taking to to A&E because I think many people would do the same! Best wishes to your DD.

Obviously she needs a full investigation but, if it helps reassure it might not be anything awful, my early periods were always full of vomiting and even fainting. Totally debilitating. There was nothing wrong and I didn't have any fertility problems. I'd definitely want my DD to have a scan though.

PID usually caused by an untreated STI such as chlamydia or gonorrhea, so I’d think very unlikely in a 13 year old assuming she has never been sexually active.

Typically presents with lots of pain, discharge, bleeding, fever, etc.

If you think it might be PID then maybe try a sexual health clinic who used to be walk ins. They'll test for infections etc. the treatment I was given for PID was shitloads of antibiotics.

Unlikely in a 13 year old.

Other possibilities:

Burst ovarian cyst
Endometriosis.

I did not have PID I had endo. Gynae waiting lists are long.

Getting her on the pill/etc may help stop the endo getting as bad if it is endo.

My DD had bad periods age 13 and I did manage to get her on the pill but it took a lot of fighting.

A and E unlikely to be helpful.

ah thanks, yes...no sexual activity, no discharge or bleeding. she has low grade fever around T38

Bowels may also be a cause.

Obvious reason 1 is stomach bug, also possible is food poisoning, could she be having an intolerance reaction? Has she eaten anything new or out at a restaurant recently?

Is anything in particular triggering the vomiting?

sorry, I should have said. the nurse we saw last night let her abdomen and confirmed the pain is gynae

the pain is triggering the vomiting I think
