What can this pelvic pain be

Have had low abdo pain on both sides (think side knicker line) and lower back pain for past six months. Also bloody knackered all the time.

Urine tests show high pyuria (pus) but no UTI. After another round of antibiotics, have finally been referred to a urologist.

bloods ok with exception of high mcv but HA125 fine, have had pelvic scan of ovaries, uterus and kidneys which was all ok.

gp doesn’t seem worried so I’ve been the one to push for referral.

has anyone had anything similar? Yes I know best answer is to wait but is a few weeks away and I’m googling with all sorts coming up.

Endometriosis? Especially if it's worse around ovulation.

I have had lower back pain, pelvic pain as you describe but right hand side only for over a year. It’s looking more likely to be endometriosis than anything else. I had ovary and kidney scans and eventually a pelvic MRI. Symptoms are worse around ovulation but it has taken me at least 8 months to notice this as for that time it was just painful all of the time. I’m 45 and have had regular, light periods for years. Next step is likely key hole to check as this is the only way to diagnose endometriosis. Get a good gynaecologist, I had to pay to see one privately as I was just dismissed as it not being serious as CA125 and scans normal but this gynaecologist took me seriously and got me into his NHS clinic which I’m now under.

Thanks very much. And apologies I had dismissed endo (in my own head) s period were fine.

My periods are regular and not overly painful but my gynaecologist still strongly suspects endemetriosis. My pain mostly occurs after period. Pelvic ultrasound was clear as endo doesn't always show up.

Don’t rule out muscular pain from the Psoas muscle which starts in the back and goes round to the front if you don’t find anything else causing the pain. I’ve had this and had a ton of gynae investigations and scans which found nothing. A good osteopath picked up on it and his treatment got rid of it. He didn’t do any manipulation but deep massage and he said all the problem was coming from my back. Worth having a chat with one if your appointment doesn’t reveal any obvious cause.

Any pelvic or abdominal surgery in the past? C-section, appendectomy etc? It might be scar tissue/adhesions. I assume PID was ruled out? No sti's?

A friend had years of urinary issues and pain and assumed to be UTI's. Then told it was all in her head and referred to a psychologist! 🙄Finally found she had endo on the outside of her bladder.

No surgery and pain doesn’t change with cycle. Is almost constant these days.

Hi did you ever get to the bottom of this? I was in A&E last night with deep lower abdo pain. Bloods all normal . Now awaiting ultrasound but I’ve been down this road before and all scans were normal. I do have back issues so wondering if psoas related. Thank you.
