Can you have lower back pain on your period and it *not* be endometriosis?

I recently had surgery to remove advanced endometriosis. All my pelvic organs were fused together. My only symptom was daily lower back ache.

Since the surgery my cycle was medically suppressed for a few months and I finally had my first period last month. It was amazing. No pain, nothing!

I am currently on my second period and I am gutted that the backache is back with a vengeance. I have heard endometriosis can grow back, my own surgeon said it was a 33% chance, but I had hoped it wouldn’t be so quickly.

So my question is, is it possible to get bad lower backache on your period if you don’t have endometriosis? I suppose the question is muddied as so many women have endometriosis but don’t know about it.

I will be heartbroken if it is back already. The op was to try to restore fertility - I’ve only had one cycle of TTC! Sad

So sorry to hear what you've been going through OP. I'm another one who had the back/leg ache and tilted uterus combo, but no endometriosis.
Hope you don't mind me adding this for those in the same position - after years of pain I had a physio who really helped reduce my symptoms through core/glute/leg strengthening work. Makes me so upset to hear about others suffering monthly pain and having to accept this when there are things that can help.

Stage 3, pelvic wall fused with fallopian tube and uterus. All if it re-sectioned.

I have constant back pain and sciatica. Also a tiled uterus.

Is a tiled uterus really common for Endo sufferers?

The amount of pain is apparently not obviously related to the spread of the endo. Sometimes women have quite mild symptoms and are riddled with it, sometimes huge pain but only stage 1. So it's quite possible you have the pain (unfortunately!) but without that meaning the endo adhesions have spread again.
