If someone is described as being really poorly what does that mean?

It generally means they are either approaching end of life, or they are desperately unwell and need surgery / interventions to sustain life.

its a friend. had a stroke. only getting up dates from family but that was the latest today.

My neighbour elderly, transplant patient years ago was in a terrible accident they thought he wouldn't pull through in and out of ICU. Too ill to move to specialist hospital. Family told to prepare for the worst. He's home now and doing well.

It really does depend on comorbidities, age as well.

My consultant described me as this with my condition, when we met a few weeks ago!!

Dd was described as really poorly in a&e. She died the next day. I totally didn’t appreciate that the dr meant REALLY poorly/ dying.

I don’t know who said it to his mum. She just said they (hospital) said he was really poorly.

Well it obviously means really unwell. It's hard to say more as it depends on a few factors.
If I said it to a colleague it would mean extremely unwell/ acutely sick.
If someone said it to a relative I would say it means really poorly but it's hard to read from that if there is prospect for recovery. Different people will use different phrases but it would mean feeling or being v unwell/ have a lot going on/ have deteriorated most likely
