Why are decision makers and executives not held more personally responsible?

Using this current news item as just one of countless examples.

So three people lost there lives in what an investigation has determined was an entirely avoidable accident. The organisation was found guilt of multiple failings and fined. Except, organisations don't make decisions or act incompetently, people do. Surely if we want to see greater responsibility then those individuals who contribute to incidents such as this should face personal accountability much more frequently? How does fining an organisation like network rail £6.7m actually give the families of the victims justice or encourage executives, responsible managers and others to do better in future?
