NHS medical records app - shocked by what I've seen

So I've been delving into historic documents on my nhs medical records app

Filed away dated in 2016 is an urgent gynae cancer 2 week referral letter - my name address phone number are is on the form but none of the reasons for referral had been ticked

IM IN SHOCK I know absolutely nothing about this! I'm wracking my brains and honestly am sure I'd remember something like this - there aren't any corresponding appointments detail records noted around that time on my app records.

I'm somewhat panicking - wtaf!!

@CopperLion someone did explain upthread and apparently it's very complex procedure for the NHS to remove details.
But you could ask for a note to be added to correct the info.
Hope that makes sense.

I contacted the Information Commissioners Office after my GP practice refused to remove the inappropriate comments from my notes. The ICO said I can add a note of amendment but not have any words or phrases redacted from my notes. Apparently to do this is illegal. As a patient I feel deeply disadvantaged and disempowered by this, whilst the abusive GP sails away Scot free.

The inappropriate comments about my "delusional" status and "unfettered access to her breasts" means I will never again subject myself to mental health services who are, frankly, a law unto themselves. There is no accountability in the medical profession whether NHS or private.

Mental health services are certainly a law unto themselves and I don't blame you for wanting to avoid them.

For posters with no experience of them - it defies belief. The most judgemental, prehistoric way of thinking, emotionally abusive, gaslighting ... Just argh. The sort of people you can hardly believe exist as they are such horrible bullies with no idea of how humans tick... And working in mental health, where they have virtually no accountability as the only record of things is what they write, and the patient can be written off as delusional/lying.

It's been a decade since I had anything to do with them... Hopefully most of the evil ones are retired or dead by now.
But that all spilled out unexpectedly as there is still some residual trauma due to their actions.

I'm so sorry this happened to you. However, it may not be as bad as it feels. What I mean is, the Dr didn't just randomly say you were delusional. He's written it's because you don't realise how unusual it is to breastfeed, and that your breasts were available at all time to feed your baby aka feeding on demand. Basically he looks like a ignorant twat. He might as well have written "this person is delusional because they change their baby's nappy when it's dirty" or something. It's screamingly obvious to the the reader that he is the one out of touch with reality!

I was recently informed at a hospital visit that my records show I had a blood clot 3 years ago ( I fainted but was just rehydrated and all further tests came back clear )
I spoke to my doctor and the practice nurse has updated it .
No one has ever mentioned this to me before !

Blimey, I would not want to look at mine. I can't anyway, as it is deemed harmful for my MH.

I'd quite like to have access to mine from, say, 1.1.2018. Relevant stuff that I might wish to correct if it's wrong. I don't want to go back to 1960 - that would drive me insane.

I don't like how blasé GP practices are about this. Records should be factual and correct. They aren't. How is this ok?
