Childhood autism diagnosis, proof as an adult. Urgent.

Cant go into too much detail, but I know someone who needs urgent proof of their diagnosis to support an urgent supported housing situation. No way of getting an adult diagnosis and no documents are able to be found.(it was 20 years ago as well, so doubt they will be accessible through the hospital.) Has anyone got any suggestions?

Do they have anything like old school reports ? Those are likely to reference the diagnosis, and might have a send statement or something with them.

It’s difficult as he is not in the area and has limited ability to contact the gp. It may be a starting point, but time is running out. No documents like school reports or ehcp kept (chaotic life). School may be a possibility. Unable to live with family.

Is he registered with a gp? If so just email them and ask for a health summary?

Write to the GP or the CAMHS team that made the diagnosis. Ask for your medical records under the freedom of information act.

His notes will be accessible by his present GP.

If it's in his medical notes his GP will be able to write a letter.

Unfortunately these can take time.

I doubt there is anything you can do quickly.

As long as he is registered with a GP then its pretty certain there will be some record of it even if its just a ‘coding’. The individual can ask for a copy of their notes but this might take time. The quickest way is still likely to be contacting the Gp and asking them to provide written confirmation, even if they are a long way away a phone consult is still possible surely?
