Grave deeds question

The deeds to my mother/grandmother/grandfather’s grave is in my uncle’s name, presumably because he is the son. Presumably they would pass down to his DD, who he is NC with.
I would like the deeds as I plan on my ashes going in there, and I assume he does too. I tend the grave frequently.
He had a big stroke, is in a home, and can’t speak. How could I get the deeds in my name ? I feel the need to get this sorted so I can relax about it.

I recently acquired the titles for my parents grave - my brothers and I were the beneficiaries of dads will, they had to sign a form rescinding all claim on the grave and I had to sign to say I wanted to take it on. The form came from the local council which owns the cemetery.
All signatures had to be witnessed as it's a legal document.
Does your uncle have cognition, is he able to give agreement in any way?

Yes, he can understand what you say.
Would it be enough for someone in the care home to witness his signature with his good hand ?

I think the witness needs to be a qualified person so if the home ever had a doctor visiting, they should be able to do it?
It's a bit like a passport application I think in terms of appropriate witness.
