What info does someone need to authorise a child's passport

DD's teacher has agreed to approve/authorise her passport but it's been so long I can't remember what info he'll need.

I only remember a few messages back & forth with the friend who did the last one.

I did one recently for a friend's child, and to the best of my recollection, I had to confirm the child's mother's details, the child's details, and then confirm that a photo that came on my screen was a true likeness of the child.

I wasn't asked anything about the child's father (parents are not together)

I’ve just done this and had to provide mother and fathers DOB, child’s full name, DOB and place of birth. I think it says it all on the website when it’s telling you you have to do it.

They need to supply their passport details, attest they have known the applicant (the adult) - personally not just professionally - for the required period, and affirm that the photograph is a true likeness of the child

They have to agree that they have know the parent in a personal not professional capacity for a number of years.

You need teachers email address.

When you complete the application you supply this and then the website gives you the instructions to follow.

I did it recently it was quick and easy.

Think it was Don and address. You just send all that to the teacher by email.

You need to have known the parent for a certain amount of time ?2 years and be able to verify the child looks like their photo.

i did a colleagues daughters one recently. We FaceTime her so I can see the 15yo version as the last time I met the child she was still in junior school.

I’ve know her Mum for 18yrs. Made us both feel very old!

the teacher taught DD 2 years ago so has known her & us for 2 years so Ive sent the info he'll need & got the text saying he'd done it
