Sore vulva - yes another thread

I posted about this a few weeks ago but did it on 30 days Only (stupid me) now it’s disappeared. I can promise you I’m not a troll. I have name changed.

Please help me. I am 39 and if this could be in any way relevant I had a Mirena inserted in August last year. That has been life changing, I had a few days of spotting and no periods since and no other side-effects to my knowledge. Around that time - and I can’t remember if it was before or after - I started getting a sore vulva. It comes and goes. I think it stops when I would usually have got my period.

Symptoms are a burning soreness, sometimes a bit itchy, but not really just more irritated. Sometimes it gets very sore and goes red, sometimes it’s just one side where the skin is quite delicate I think from where it was injured when I gave birth. Otherwise there is nothing else to see down there. It tends to get irritated by sex. Sometimes I get a burning pain when I pee, but that doesn’t happen very often.

I went to the GP and she said the skin looked a bit sore and split, took swabs for herpes which were negative, told me I needed to look after the skin. I then used aqueous cream, I don’t use any products on my vulva except water, I’m using sensitive washing powder now, cotton knickers, going commando as much as possible (overnight etc). I even switched to more soft toilet paper to no avail. I’ve never had sensitivity problems before.

My GP initially gave me steroid cream, that didn’t really do anything, I went back and she gave me oestrogen cream but told me she thought it was highly unlikely that that would work. I tried it for a few days but also used canaesten cream - I do think things got better but not sure if was just my “period”. But then I thought it was the thrush cream - this is because she did the initial swabs for herpes but also, the other one for thrush which subsequently showed a light covering of thrush (for some reason this didn’t come back at the same time). I then decided it must be a recurrent thrush infection after reading some of the threads on mumsnet, even though I didn’t really have discharge and it didn’t seem like thrush from when I’ve had it before. I’m currently taking the oral and pessary treatment, but if anything it’s become worse. I’m sore in the morning from the thrush cream irritating it overnight.

GP thinks I’m way too young for it to be menopause related etc. I don’t know whether I should be getting back on the oestrogen cream or what.

The only other thing I can think that’s possibly relevant is my DH has had itchy testicles which gp said was jock itch. He also had an itchy bum at one point and gp gave us all a worm tablet, no change. Sorry I realise it’s grim but at this point I’ll talk about anything if it stops it. We also don’t have sex a great deal - we might if I wasn’t so bloody sore.

It’s not an STI from a third party - I know some people will jump to that conclusion but I promise it’s not.

I am at the end of my tether and really just need MN wisdom on what this could possibly be? I feel like I’m bugging my gp and I live overseas where I have to pay each time I go. Please help MN.

(Just to say though that even if you do have a progesterone intolerance you still need to take it as part of HRT if you have a womb as it stops the lining of the womb from thickening which happens with oestrogen use - but there are different ways and types of using progesterone. I know that’s perhaps not relevant to you but wanted to post that for the benefit of anyone reading the thread. You can’t just not have the progesterone part of HRT if you needed it).

Agree it could be progesterone intolerance.
I have lichen sclerosis and you could have been describing my experience in your first post (though I don’t have a coil). With untreated LS you often get tiny fissures in the skin which can bleed a little.
steroid cream works well though it still flares up, particularly with stress.

Two years ago I was diagnosed with lichen planus. I had had all the symptoms you describe. Gynaecologist prescribed elocon steroid ointment to use for two weeks 2-3 a day and zerobase cream to put on every morning and for showering permanently. This has helped a great deal and now I only occasionally get a mild burning sensation which I can easily remedy by using the zerobase.

Ask for blood tests!

I had this for the best part of 4 months, nothing was helping, it was driving me mad!
I also thought possibly early menopause ( I'm 39) anyway she took blood tests and a swap, turns out I have type 2 diabeties and apparently this was the one and only symptom I had.
Since cutting out sugar/ cards things have returned back to normal.

I had similar issues for months recently OP. All swabs from the GP came back negative for STIs etc. Turned out to be Aerobic Vaginitis caused by a laundry detergent. Have a look and see if the symptoms fit. Mine was treated with Clindamycin which you can get at Boots online pharmacy

Taking into account your DH has problems too, I'd guess fungal
Try Flucanazole oral tablet and see if it helps

@Newname10 how did you get your aerobic vaginitis diagnosed? No Gp or gynae I spoke to had heard of this condition. I am sure it is what I have.

I bet it’s thrush ,and your partner keeps reinfecting u when u have sex .
i tried some antidepressants and they gave me thrush and cystitis at the same time
I needed 3 over the counter thrush treatments ,2 pessarys with external cream rubbed in twice a day and one oral capsule to actually shift it…and that was with no sex .
took 2 weeks of no swimming and the medication before I was back to normal .
took ages to shift ,

Hi OP, in short I didn’t. Eventually researched treatment myself and found that most cases of AV are responsive to Clindamycin. Got a course for 7 days from Boots online pharmacy as a BV treatment and it’s been life changing. Symptoms pretty much resolved in 4 days. I made sure it wasn’t any other vaginal infection first as I’d been tested for everything by the GP before ordering (all STIs, strep etc).

I would encourage you if you’ve been given the all clear for other infections to go ahead and order via Boots online pharmacy. You need to say you have BV symptoms to have the prescription approved.

I swear mine was caused by Bold laundry detergent (and will forever live in fear of it now)!

Hi all. So I ended up seeing a different GP who did another herpes test as it looked “very red”. That was negative and she thinks it is most likely vulvovaginitis from my Mirena. I’m gutted. Mirena has been life changing so it’s either suffer or have it out. Don’t suppose anyone has any advice? 😭

I think you should go back to the oestrogen cream, Ovestin, and use that for a significant period- it won’t work straight away, I think you gave up too early before. If that makes a difference you might not need to remove the mirena.

If I’m being honest not long - 10 days? But my gp was adamant I’m too young. I’m only 39 no other signs of menopause.

39 really isn’t too young to be low in oestrogen / peri menopausal. Your Gp is wrong and you need a second opinion. Even if they agree to do blood tests it won’t be any good as hormones fluctuate so much during the peri menopause anyway - specialist clinics like Newson health prescribe HRT based on symptoms alone. You have symptoms, you need the Ovestin. And you need to use it a lot longer than 10 days to see any results. At least a couple of months.

I think if you give the Ovestin a really good go, every day for 2 weeks and then 2/3 times a week and externally as needed for at least 2 months and you’re still having issues you need to seek a specialist referral.

Try trimovate and Lotriderm cream

Also fluconazole or itraconzole 2 or three tablets a week apart.

If this doesn't work then it's not thrush.

Once tablet per week for three weeks. Also vagisil has an and the tix in it so you can go and buy that from the supermarket

Op, you need estrogen down there. I was a similar age to you when I started using vagifem and ovestin. I was also very red and irritated. It took almost a year for improvement.

The coil can harbour bacteria, especially bv and thrush. I know quite a few women who have had uti like symptoms and random vaginal infections after having it inserted.

I had this for over 3 years. I had swabs, bloods, scans, creams, steroids, changing diet, detergant, underwear, bicarb in bath and nothing would shift it. I'd had enough of being fobbed off everytime I went to the gp or gyno. A new gyno took over one day as mine wasn't there and she put me on amitriptyline and said she doesn't know why it wasn't done sooner. It's a 4 month course and I'm on my first month and haven't had it since. Speak with your gp about it. I really understand how your feel its horrible. I was so sore some days I had to sit on ice!! I could barley walk because it was so sore and swollen. I'm not sure what it was or what caused it. It's definitely worth speaking with your gp again. You shouldn't have to suffer.
