Can't face AIBU as I'm feeling fragile but back in July I had a text asking me to phone for an appointment for my adult ASD dd for a medication review. I phoned on the day got a telephone appointment for yesterday. Was told they wouldn't be able to specify a time, Doctor would ring once, if no reply I would need to make another appointment but that would mean that repeat prescription might not be approved. Calls were to be any time between 8am and 6pm.
Waited by the phone all day no phone call. Contacted surgery today asked about the missing phone call, she checked records and said she would contact GP as there was nothing there but if I wanted to speak on dd's behalf I needed to fill in a consent form.I have to speak on her behalf because she can't speak (situational mute) Not a problem, asked her to post me one as I don't drive (partially sighted), so when they closed the satellite surgery in the village it means I now need to take two buses to get to the new swish surgery and it would take me two hours to get there and back. I have two disabled young adults who I can't leave for more than an hour and they won't use public transport. They won't post and won't give them to my other daughter as they have to see me pick them up. They then contacted me again to tell me that GP said medication review was unnecessary so he hadn't phoned but they would contact me in six months to arrange one then.
I've emailed the practice manager not a formal complaint as such but asking whether they can come up with a solution as to how I can get the forms. I can't believe how difficult this all is. I would move to a closer or more accessible surgery if I could but there are no spaces. I can't believe how difficult everything is.

Could they email you a copy that you could print out, complete and send back?
It all sounds very hard work though -and it really shouldn’t be-why are they just asking for a form now when, presumably, you’ve been speaking on DD behalf previously?
The Dr should as have at least sent you a text message saying no call needed & they would send the prescription to the pharmacy.
I wouldn’t bank on them somehow remembering to contact you in 6 months. Stick something on your calendar and maybe do an EConsult in 5&1/2 months to ensure you get the prescription in time.
sorry it’s been such a headache for you.

Dd is 19 and a half I've been speaking with Doctors on her behalf as I have my son who is 27 without any issue previously with no need for this consent form. If it was online I would have done so I checked all the forms they hold but it's not online because they need to see me pick it up to confirm that I am the person they are consenting to speaking on their behalf. They have been the dc's doctors their whole life they know who I am and where I live so surely if they post to me at the address they hold, the same address as the people giving consent that would be enough. It's not as if I'm ordering or picking up controlled drugs and besides which I can order and pick them up without their consent I just can't change the pharmacy to one that is convenient that I can get to within the hour I can leave the kids without there being a problem outside of twenty texts asking where I am and how long will it take for me to get back.

It seems unnecessarily convoluted. Different surgeries hold permission for me to speak to them about DM and DD and that was on the basis of a letter for each.

Complaint to the practice manager would seem the way to go to begin with, possibly mentioning reasonable adjustments for you to access services under the Equality Act.
