Hair loss - sorry, long

My 22 year old daughter has been suffering from hair thinning and hair loss for over a year now. Her density has gone down so much (probably half of what she used to have) and she has hair fall every time she runs her hands through her hair not to mention how much comes out when she brushes it, she is scared to brush it now.

We have seen three different dermatologists which have all diagnosed her with different things leaving us feeling very confused and frustrated.

The first derm diagnosed her with Lichen Planopilaris triggered by COVID, saying that he can see scarring towards the back of her scalp which as you can imagine has been so devastating for her. He booked her in for a biopsy to confirm this diagnosis.
With the second derm the experience was extremely different. My daughter has PCOS, the derm pointed out the mild acne on her face too and said she suspects its a hormonal imbalance causing Female Pattern Baldness and she prescribed Spiro. When I told her about the previous diagnosis she told me that she sees 0 scarring and that what my daughter has is classic FPB symptoms and no biopsy is needed.
Obviously not being satisfied with two completely different diagnoses, we went to a third derm and she gave me yet another diagnosis, Telogen effluvium from COVID and mild FPB and prescribed topical minoxidil.

It just baffles me that three qualified dermatologists can have such different opinions. Its also extremely upsetting because we have spent so much money just to not have a solid diagnosis. We really want to know what is the actual reason behind the hair loss because we can’t treat it properly unless we know what’s going on. I also don’t want her to start any drugs just for the sake of trial and error and without knowing what this is.

Does anyone have any experience/advice regarding this? I am really torn on what to do.

Has she had her thyroid levels checked? My hair thinned massively when I developed an underactive thyroid. Unfortunately it's never thickened again since.

It’s a common symptom of PCOS so if she already had that diagnosis I’m surprised she was even referred to the derm. Has she had her hormones checked recently?

My husband and mother in law have alopecia universalis. They've both lost all their hair.

Various doctors have said various things over the years, but the basic cause is that their immune systems have gone nuts and attacked their hair follicles.

There is no treatment for it, it's just one of those things.

If blood tests come back clear, I'd start spending some money on things to help with her acceptance of the situation. My MiL wears lovely human hair wigs, she also had her eyebrows microbladed.

My husband just rocks the skin head look but it did take him a while to accept losing all his hair.

Does she take any hormonal birth control? The mirena led to terrible hair loss for me.

Why did you go to another derm before having the biopsy?

I have PCOS and have the same. My hair is thin all over and the hair on my crown is much shorter than the rest. It's improved drastically with Dianette but it took time (months not years) to improve. It will take time with Spiro too.

Oh and minoxidil should be avoided if possible. Works brilliantly but can't be stopped without an alarming amount of hair loss.

Can I ask if you considered a trichologist?
I ask as I'm experiencing the same at the moment (I'm 48) . My doctor grudgingly ran some blood tests and everything apparently was normal so it's just 'stress' according to her, not peri menopause either..... Her only treatment advice was 'counselling '.... Well if it is stress, I'm making it ten times worse as it's massively taken over all my waking thoughts!!
There was an article in the new Sainsbury's magazine this month, weirdly, which recommended Trichologists so I've made an appointment, as at least it's something to try. Like you say it's so frustrating not knowing the cause. Did the doctor recommend dermatologists to you for a specific reason?

Thank you all for your replies and suggestions. We decided to do extensive blood tests - trying to do it via NHS and take it from there. I'm also convinced that whatever it is, a wholesome approach would be the best. Hair loss is a symptom, very distressing one, but something else is not right there. Perhaps we should look for a functional medicine doctor, not sure.
I just want blood test to be done asap.
