ADVICE - Applying for a Grant for Disabled Facilities - Means Test Form to fill in!!!

In a nutshell...family member in hosp but needs house adapting for them to return to - big job as an extension or some room needs to be built for them as they have no capacity as the illness has slightly damaged brain and at mo can not walk.

Spouse of family member has forms to fill in to apply for grants to help fund - it's means tested so they want to know about incomings eg. pension, savings etc, in both bank accounts

Any advice would be much appreciated.

It just means savings, state retirement pension amounts , occupational/ private pension amounts, ISA's/ premium bonds etc or anywhere else where capital is tied up. Does the disabled family member get a disability benefit currently ? if not you should apply for that for them. It will be Attendace allowance if over state pension age unless already getting pIP or DLA.

the family member is in hosp and no nothing has been applied for yet. We are waiting on a meeting with the hosp and was going to ask for contact with a social care worker as all this is a shock and obvs now the family member wont be able to return to work or the life they had before. So nothing coming in at all

It would be very unlikely to have the building works complete before family member is discharged.

I have known this to happen in very extreme circumstances but usually hospital will discharge to a safe place.

This could be home with one room for strip wash and commode. Called a micro environment.

Meet with hospital but usually people are discharged with care package and appropriate equipment before starting to assess for long term needs (building adaptations).

Assuming you're in the U.K. and relative is in acute NHS hospital Smile

Yes we understand and hoping for member to go elsewhere beforehand as there is no way the property can be adapted beforehand but just need to get the ball rolling etc. very sad situation really. Member cant walk or get to a toilet...also property is a very old cottage so really difficult to adapt

I guess they may have to go into a temporary placement in a care home whilst their own home is being adapted, but that would be for the hospital discharge team to sort out. If they are working age, and have no income coming in and savings less than 16k, you need to get the ball rolling with claiming Universal credit now as that can be claimed whilst the person is in hospital and claiming PIP once they have had difficulties for 3 months.

Persons been in hosp for 6 months already!!!!! We need a social care worker to help, sadly so many failings with the hospital to date that we just dont know where to turn for advice!

Oh gosh that sounds tough. I presume you have spoken to adult social services yourselves, or the hospital discharge team should be liasing with them ? i would demand a meeting with the hospital discharge meeting to see where things are up to.

Complaints and PALs communicated to since January onwards due to several failings. We asked for an urgent meeting with lead consultants and a second opinion - second opinion was flagged up by us because....the consultant said they dont know what's wrong with person who has been left brain damaged by 2 infections - one that developed whilst the family member was in their care in hospital and has cause detrimental damage - consultant said so they can go home!!!!!

We were told yes absolutely we will help facilitate the second opinion and come back with a date for the meeting - that was 5 weeks ago.

Sounds like a nightmare ! I'm hearing of situations like this again and again in my job role. It just seems like the whole NHS and social care sector is overwhelmed. I hope you make some progress soon.

Most Councils will have someone who can help you fill in the forms.
If not, it's fairly straightforward if you have a regular income / standard savings - and the Council will work out how much grant you can get.
