Torn ACL 2nd opinion

Not sure where to put this... I'm hoping this is where all the ortho surgeons and physios hang out on a Weds morning 🙄Grin

I ingured my knee 8 weeks ago, badly. I have hypermobilty and twisted/popped/dislocated standing down from a large step. My MRI results are below.

I can't weight bare and my knee has kinda locked in a slightly bent position and I've lost muscle mass. I'm in a lot of pain.

I saw a surgeon yesterday who told me I need intensive physio to rebuild the muscle, fully accept this and prepared to put in the work.

I was then told that I would not need surgery to repair the ACL.
Now, I'm no surgeon... But my understanding is (I've been reading/researching over the last 8 weeks) that a fully torn ACL will not repair in its own without surgery. There are obvious risks to any surgery, but the cons of NOT having surgery are very worrying to me; continued instability could cause further accidents and increased chance of osteoporosis developing etc

I fell because of my hypermobilty, most of my major joints have popped out over the years, some are already fairly unstable but I manage it well and know my limitations. Although not diagnosed I am almost certainly at the begining of osteoarthritis in some of my joints also.
Despite this I have a full-time physical job. I am otherwise fit, healthy, good weight and fairly strong.
I'm desperately worried that physio isn't going to be enough to get me back to full fitness and mobility. I'm 46.

Anyone know whether I should be pushing for a 2nd opinion and ultimately surgery?
I've got my first appointment with the physio tomorrow, so will speak to them obviously.

Anyone with experience of this or advice? Tia

Sorry to hear about your injury. I know this is an old post but how are you feeling now op? I have a similar injury so I was scrolling the boards. Did you get surgery in the end?

I know this is a zombie, but I'm also hyper mobile, had a total ACL rupture and had to have an autonomous repair. I was told there was nothing but mush in my knee, and it would never be stable enough for me to walk without the surgery. I did 3 months of intense physio first to build up the muscles, but had surgery 10+ years ago. It definitely made it possible to lead a pretty normal life, but it's not perfect. I'd go for a donor repair via keyhole if you can - there's no betting that your tendons are any better than your ligaments if you're hyper mobile.....

@JingleB3lls I had a ruptured / torn ACL if that’s what you have. I was told that I could do without it and do lots of physio instead. That was complete bs and I had an op to replace my ACL with some hamstring. Luckily private before Covid.

that was 8 years ago. The op was a success but I do a LOT of exercise like squats and lunges, and when I don’t then my knee quickly goes wobbly.

I don’t think ACL repairs are easy, and they require upkeep.
