C Section - Bad Infection for weeks and still ongoing

Hi, my wife has been in hospital (NHS) with a bad infection/fevers for almost 3 weeks now. She gave birth 6 weeks ago via c section.

She was initially admitted into hospital around 3 weeks ago and diagnosed as having mastitis and endometrosis and given antibiotics. The mastitis cleared and she felt better for a day and was sent home. As soon as the antibiotics finished however around a day later, she immediately got fevers again so we went back to hospital.

She was put on IV antibiotics again, none of which worked and continued to have bad fevers. We were then advised she would need to have surgery following on from a UV scan where they found what was believed to be a pocket infection.
They opened her up again and to our disbelief following surgery we were told they found no pocket so they just cleaned the area instead and left a drain in her to clear any other fluids and advised us they thought it was some swollen muscle! It was all very confusing and unclear but we just hoped she would get better anyhow.

They continued with antibiotics and again a day or so later she became very Ill again with temps of 35 one minute and 39.5 another! All of this while under immense pain from another surgery 3 weeks on from the first!

We were not happy as the antibiotics were not working and she was suffering so much so they then ran a CT scan and found another pocket (I think they missed the first one) and ran an IR procedure to drain it. The doctor who ran the procedure said the pocket was very deep behind the main incision which again leads me to think they missed it on her surgery the few days before!

The procedure was completed and samples were sent off to the lab to determine what bacteria was growing inside her. This was last Wednesday and they havent been able to grow anything and tell us what exactly is causing the issue!

They swapped her IV antibiotics to menoparem? which reading her notes was recommended if she went septic!! Her fevers seemed to have gone and she is/was feeling a bit better in her self however, we were discharged yesterday and swapped to a new set.
We noticed when we got home the new oral antibiotics were the same ones co-amoxiclav that she was prescribed when the issues started.

Prior to allowing her to leave they done another UV scan and advised 2 pockets were found! They said they were very small and should clear up with antibiotics and she was so keen to come home and be out of the hospital so she is now home.

The Dr told us we need to go back in a week to have another scan to ensure the infection pockets havent grown!

I am of course massively concerned as my wife has been through so much now and has missed weeks of bonding with the baby. She has lost so much weight and is still very fragile and in pain!

She has literally been like a Guinea pig at the hands of the NHS and has suffered more than you would wish on your worse enemy. I genuinely thought she was going to die. She has had blood tests daily, multiple catheters some being installed after 3 attempts 🤦‍♂️, at least ten I’vs being put in her arm, 3 surgeries in a month, antibiotics and morphine daily and somehow still manages to keep going and is my hero but I just want her better and enjoying what should be the most amazing time of her life. We waited years for her to become pregnant so its so heartbreaking to see her this way.

This seems to be quite a rare and severe infection so I wanted to check if anyone else here has has a similar issue and what it ended up being as the NHS still haven’t fully diagnosed it after almost 3 weeks and I’m trying to do so myself now without much luck!

Sorey to hear all this, she has been theough an awful lot. The blood and wound results must have shown what infection she had and which antibiotic was the most appropriate.

Meropenam is used for a variety of bacterial infections including sepsis. Did she suffer with sepsis.

No it was used as her infection / wound area became septic but they have not mentioned Sepsis. This was ruled out early on I believe in the process.

These are the discharge notes" "Mrs * was re-admitted on 1/3/22 presenting with abdominal pain and infective symptams S weeks post- section (28/1/22). She had previously come in for Mastitis and endometritis for which was treated with antibiotics. An US-abdomen was done on 4/3/22 which showed evidence of a collection for which she underwent an EUA, exploration of wound and washout later that day. A drain was left in-situ post-op to monitor and drain fluid culture was sent to the lab which showed no growth of organisms. She had been catheterised and a CT-urogram + Cystogram was done in order to rule out bladder injury/pathology (suspected as drain fluid seemed to contain urine) which was normal. Blood cultures taken on 9/3/22 were negative. Her blood tests showed inflammatory markers improving and eventually became clinically stable. She had started on Ceftriaxone + Metronidazole + Gentamicin and as per Microbiologist advice we hnd switched to Meropenam IV (as spiked fever couple times) and then later switched to PO Co-amoxiclav on discharge. She was given TEDS + LMWH during her stay as VTE prophylaxis.

We have been told if the scan next week shows the collections growing she may need readmitting, more IV antibiotics and possibly another procedure to drain them. We are praying though the antibiotics work and she gets better.
