Incorrect name NHS

DD changed her surname via deed poll 18 months ago, told all the relevant people and provided all relevant paperwork.

A while after that we received a letter from the GP in her old name. Rang them to confirm name change, they said new (correct) name was the name on their system so we thought all was fine.

Received a letter about getting a vaccine recently, again, in her old (incorrect) name.

Contacted GP again to check what name they had and they confirmed that they definitely had the correct, new name.

Have just created an NHS login for DD as she will need a covid pass in a couple of months...the records are showing as in her old name!!!

Rang the surgery again and they confirmed they have her correct name on her records.

I'm not sure what else I can do, obviously her NHS records need to match her passport.

Has anyone experienced anything like this?

Is there anyone else I can contact about her records? Everything just points me to the GP surgery and they say they have the right name Confused

I could be wrong but I think the vaccination letters are sent our by the local health authority and not your GPS surgery. Might be worth looking into?

It sounds like there are additional departments you need to update, contact PALS, they should be able to help you.

I agree contact PALS and it may need updating at the local hospital as well.

Ok, I'll email them.
When you're given advisce after changing a surname it simply states inform your gp and they'll change your records.
I know when I changed my name after getting married everything I received after that was in the new name.

Did she have written permission from everyone with parental responsibility? If not, they won’t accept it. Could that be the problem?

I changed my name 4 years ago, updated everything and despite constant reminders they are still getting it wrong!

Yeah, done with all permissions needed, only adding a '- extraname' so not a complete change. Signed letter of consent etc. And they've accepted it because according to them that's the name they have. Weird.

You need to make sure they've changed her details on the spine. It's possible to change details at a local level on some systems, but the spine is centrally where info is pulled from.

I've just checked her name on the NHS login details and it's now correct. I haven't contacted anyone other than our GP surgery yesterday Confused Grin at least it's sorted.
