Is anyone a GP or works at a GP surgery ?

Are things coded sometimes on medical notes ?

If so is there a list I could access

Or can you tell me the code for ‘self harm’ and are there any similar codes for things ?

I think there’s been an admin error somewhere as I have self harm recorded in my notes and I’m struggling to find out why, trying to get it removed and wondering if some kind of coding error is the reason ?

I'm not but there was a similar thread not that long ago about something that had been miscoded/ recoded and lots of helpful people came along to tell the OP what the codes meant.
I’ll try to find it thanks. I haven’t seen any codes on my online record but going to get the paper ones as I can only think this may be the reason of something has had the wrong code put on ?

I haven’t seen that but I’ll check when get records , I’m wondering if there’s something I do have and the code is similar maybe to the code for self harm and that’s how it has got on my notes ?

A code is typed in e.g. #BM12, the corresponding clinical entry appears on your record. It doesn't show #BM12. Unless you mean an abbreviation like DSH, deliberate self harm, mentioned above.

Oh , yes I thought it might just be a code and that could be where the mistake has happened

I work in a gp surgery
There are millions of codes and yes there are some for self harm. Notes are mostly free texted with codes in it. Ie "high blood pressure" coded, then info regarding BP readings, lifestyle etc. X

@AlmondcroissantAndLatte there are thousands of codes for all types of things including medical conditions (e.g. asthma), mental health conditions, things such as "consent" or "has a fire arm" or "number of units drank in a week / cigarettes smoked) etc. They reason they code is so that if reading back on medical records and you want to just look at specific consultations to do with a certain thing, then you can. Things are also coded so that "QoF points" are achieved.

It is possible in consultations that an incorrect code is accidentally added, as sometimes you can be typing and a code comes up, one wrong click and that's it. Obviously it shouldn't happen, but it does.

They should be able to look back in your care history to see when the code was first added / what consultation it's linked to etc.

@AlmondcroissantAndLatte we code on patients records who has picked up the script and type in a code which brings up "prescription collected by patient" however my slapdash colleagues sometimes don't type it in correctly and don't bother checking before saving and it's come up as "occupations". I really do despair and and I'm totally frustrated in work as no-one seems to care when errors are made.

If there is a mistake on your notes just ask your doctor to rectify it.

There have been cases where a previous code and changed into something different during a software change. For example, we had a problem where patients were originally coded as having had a 'tremor' but it changed later to 'Parkinson's Disease'.

It could be as simple as a code for 'intentional laceration', 'intentional drug ingestion' or something that has now changed to something else. I have made these up, by the way, the surgery should be able to tell you. My colleague is the data guru and can always find the rogue code.

I think it must be something like this it seems the obvious explanation I’m just hoping I can find the error and get it sorted out

My notes which I accessed on GP app say I have stage 4 kidney disease. I've never had a problem with my kidneys! My mother died from kidney disease, but I can't see how they could have muddled up our notes as we went to completely different GP practices. There must be so many inaccuracies.

That's worrying as things like this can affect things such as life/ travel insurance etc. It could be they tried to code family history of kidney disease. ? Def get this changed!
