Painful nail infection - best course of action (sorry if TMI)

For a few months I have had (what I'm sure is) a fungal nail infection. I've had one before and used an OTC treatment and (eventually) it grew out/cleared up. This one seems different, more cracked/crumbly (ewww sorry!) and situated at the edge of my toenail. I've been using a different OTC treatment but the skin at the edge of my nail is quite puffy and red, and it's quite painful, even when not putting pressure on it. (Thats why I'm awake at 4am!!)
What would you say the best course of action is? DH says go to the chemist "and get some cream" . I'm dis-counting this advice as I am already using an OTC remedy and it's not so far effective. I've just made an online enquiry with a "foot health practitioner" at a local clinic...but should I make a gp appointment? Ideally I hoped it would go away with the nail varnish type stuff, like before, so I wouldn't need to show anyone Blush but because it's really painful (I've been awake on previous nights and it hurts during the day) then I feel I need to be more proactive.

I had a persistent athletes foot for years that nothing would shift. Nail became yellow and flaky, though not painful. I ended up going to my go and getting a course of pills (don’t remember the name, sorry) which I took for at least a month. All cleared up and nothing since.

Creams and unctions didn’t work. Suggest you ask either pharmacist or go about a pill treatment asap.

Sounds like it might be infected? I would either see your GP or a podiatrist.

(Not sure if that is same as a foot health practitioner?)

I have a long standing toenail thing as can't take oral drugs for it as I'm on treatment for rheumatoid arthritis - but it isn't painful - just gross and mildly annoying Confused

Plus - don 't be embarrassed about it - they will have seen much worse.

Definitely GP.
If its infected you need antibiotics.
I have horrid crumbly, disgusting toenails, I just paint them, but they're not infected.

Can I ask who gave the diagnosis of a fungal infection this time?
Without an accurate diagnosis you can't have an accurate treatment.

I would suggest you see a Chiropodist/Podiatrist for evaluation of the situation.

A Foot Health Practitioner is not the same type of practitioner BTW
