Has anyone changed their name at the surgery and how long did it take ?

With me it was instant. I had tacked my husbands name on to the end of mine when my kids were babies and asked them to remove it. There was a bit of an argument as they told me I needed a deed poll! I explained that I didn’t and they went ahead. Maybe ring and ask what the process is?

Will do, thank you, just fretting a bit before the surgery opens tomorrow.

My CV card is in my married name, my passport is in my maiden name, I only changed when DS was little because the very lovely but very twinset and pearls receptionist couldn't get her head around the two different names.

It has never been a problem up to now, but we have never been in a pandemic before, Grin so I need to go back to my maiden name and get my CV card issued in my maiden name and then I can apply for the app to go home to Dublin.

I haven't seen some of my family for two years and we lost an old member and a young (55) member to covid, so I need to go home and if nothing else close that grieving circle.

Sorry to hear about your family. I don’t know how long it takes to cut across to the NHS main computers or whatever the information in the app is pulled in from.

DH is going to Ireland this week and he said he had downloaded a letter from the NHS website which is linked to the app. I don’t know if they accept the little card.

I change names. After we have seen proof it's more or less instant. You need to show deed poll or marriage cert. if going back to maiden name, your birth cert.

You should be able to download the
NHS app now. It asks for your NHS number and date of birth. Your vaccine info should be on there already. You can generate a letter from there once your name change goes through.

Just re read the post. It may take a few days for nhs app to catch up with your gp records

My surgery wouldn't do this. I'm taking my marriage certificate as extra proof instead and the place I'm going also allow a lateral flow test so am doing one of those too.

I don't think the surgery ever said anything when I said I married dh and was changing my name to his and the kids. They just did it.

I have been on to them they want my passport and wedding certificate and they are happy to do it.

I haven't seen my mother for a year (did a mercy dash home legally, through Belfast) when we thought she was dying. I saw my siblings and their partners at the time, but not my nieces and nephews.

It is Mum's birthday on the 6th, I land on the 6th she doesn't know. Grin
