Who should I report dangerous car repairs to?

To cut a story short:
Car went for MOT.
They took wheels off for a brake inspection and to upsell brakes.
Refused brake work, collected car, drove 40 miles, wheel fell off.
RAC attended, none of the wheel nuts had been fastened.
Car back to MOT place for them to inspect damage and replace wheel etc.
They changed the brakes and nothing else Hmm

Now convinced it's not merely a mistake it's negligent so who do I report motor safety to?
If anyone works in the industry even better!

I think it's VOSA who deal with MOT's.

I've googled and thought VOSA but I want the Health safety equivalent so checking with the wise folk here.
There were no injuries fortunately just vehicle damage but at the first opportunity they did the brakes so I'm left thinking either A. They lied on advisory about brakes B. Brakes should have failed MOT or C. Nothing wrong with brakes they wanted to sell brakes.
It's a national chain but there is summat fishy.

That should be DVSA, I googled and thought DVSA.

I'm pretty confident it will go to motoring ombudsmen I don't think they cover safety though.

At the moment Lindt they are furiously trying to control things (it's DSon car and he was driving) they are promising the moon on a stick so I'm getting battle vibes.
Not a thing in writing, no email nothing.
I'd better wash my Judge Judy costume.....

We just had an RAC vehicle inspection and the MOT centre have damaged the steering and power steering when they fitted the questionable brakes. It just gets worse they sent him away in an accident waiting to happen.

Honestly Prof I'd heard bad things about the quickest fitters so I told DS to avoid them Angry I didn't know these Orange competitors did MOT I thought they just had retail that sold pushbikes and bulbs.
I'm really cross.
He had his lights checked the week prior at the retail store where they booked him in for MOT.

Cheeky fuckers upsold him all the way. I'm furious with myself too I was teaching him a lesson, thought they would charge top prices but not fucking this

For free Hmm
After RAC guy made it safe he drove it home and I called the next morning (DS at work) they asked for it back to fix any damage or at least look at it.
When we both went the next day to drop off car the manager (almost crying, seriously) agreed to fix Everything we asked for, including hire car because some arm was damaged, and an email apology to sons gf who was passenger.
They called after five hours so DS went to collect, all they had done were the brakes.

This DS is shy, driving miss daisy type. He'd not gone over 32mph.

Then quite likely your number would be up. Although I'd hope you'd have noticed before you got to 70 there was something wrong and stopped. That's usually what saves most people from a catastrophic failure.

I knew of some speedy fitters (to be fair it was their mobile outfit) that managed to leave wheel bolts loose. The car quicker became so obviously wrong the driver had to stop and call the RAC. Who were not in the slightest bit surprised. ("Can you do me a letter to say what happened ?", "I think we've a few in the office").

Two of the other wheels only had two bolts in.
It fell off on a busy roundabout. Rolled off down a hill!
Almost all his journey has traffic calming and humps, plus no uni.
The RAC fella did a really detailed report thankfully.

The orange people have had all weekend to arrange a loan car and collect DS damaged car. They've ignored all contact.

Is the next step put it in writing and ask them again to put it right or can I just inform them we are taking it to dealers?
Am I right thinking they had the first opportunity when they changed the brakes and reasonable opportunity over the weekend?
DS is working nights so having to try and scrape lifts.

Contact Trading Standards.

I wish I had done this but I was young and it was my first car, bought from chain dealer, had issues with car, had it checked by my friend's Dad who liked to tinker with cars, he said it is a shit car, get a professional report on it. Booked RAC to come out and assess the car, they were appalled I had been sold this car with a "100 point check" before they hand over the keys.

Garage fell over themselves to shush me, upgraded my car for free but I wish I had reported them.

Just got a response from DVSA....

Thank you for your email dated 23 October 2020.

DVSA have an Intelligence service where reports are submitted and may be investigated by our Regional Intelligence Unit (RIU).

Information can be left anonymously via the email address [email protected] or by using our Freephone number 0800 030 4103. Our opening hours are Monday – Friday 07.30-18.00.

To make an intelligence report could you also please provide us with the name and address of the garage you are reporting.

Still waiting on trading standards reply.
The orange ones now have the car, they dropped off a hire car at DS work and collected his car from home on a transporter.
Despite repeatedly mentioning he didn't want car to go to the same garage (DS clearly said he had lost confidence with them) I've driven round the shopping area it's located in and sure enough it's there.

Still nothing in writing or email. Sad
