NHS Flu vaccines at Boots/Lloyds?

How does Boots or Lloyds (or other pharmacies) know who is eligible for an NHS flu vaccine?

I am eligible for a free vaccine because of a medical condition but my doctor's surgery slots are completely full. I know you can get the NHS vaccine at a pharmacy but I'm just wondering if you have to bring some kind of proof or are they able to access your medical records or something?

You complete a form and take it in with you. Specifying why you are eligible. I assume the NHS do something spot checks same as they do for exempt prescriptions.

with your permission, they might be able to check your summary care record but otherwise you would fill in a form and they'll take your word for it, then it'll be checked when they claim a fee from the NHS.
