Help where do I stand with this work situation

I work as a carer. I work for a family that have a company that does HR. During he last year I have been unlucky health wise. Miscarriage, stomach ulcer, thrown back out etc.
Up to now work have been really great as I really go above and beyond and quite often do hours at very short notice. I have never had a back to work interview
2 months ago a different company who does HR and pay roll took over and I was asked to provide a list of when I had seen the doctor.

I requested this from the doctor straight away and was told as long as I provide that list everything would be ok. Quite often I was not off long enough to require a sick note so they just want to check (fair enough)

The problem is I can’t get this information out of the doctors. I requested it six weeks ago and was told it would be ready in two weeks. I have chased it every week.
This morning I had a call from the HR company saying if I did not provide the Info in the next couple of day’s they would class my absences as not proven and a decision made about my future.

I have just the doctor earlier and it is still not ready. It is just basicly a list of when I have attended that is all it is. I told them my job is on the line here basically in tears and they said they would chase it up and get back to me in the next hour. No phone call and now the surgery is shut this afternoon for training which they did not tell me about on the phone.

I don’t know what to do. I know doctors surgery’s are busy but they said it would be ready in 2 weeks six weeks ago. I could lose my job over this.

Tell the dr’s that you will report them to the ICO. You have made a Subject Access Request and they should have responded within a month to either supply the information or let you know they would need an extension.
I would do this in writing and give your HR the same letter, so they can see you have been doing something.

Have you signed up for Patient Access/EMIS? You can log on and print it all off yourself.

I can but they also want the reasons I came to see the doctor and patient access does not cover that

It does on mine. It says appointment date, time, doctor's name and "problem" e.g. "problem: ear ache," "problem: shortage of breath."
