3YO with awful night cough

One of my twins has had a cough through the night for months. We have a constant stream of colds and bugs from nursery so I haven’t thought too much of it but last night was awful - he was coughing for hours, it’s a dry cough but he was coughing so much he starts gagging. Last night was the first time I really thought this is not right at all.

He’s constantly got a runny nose, and breathes through his mouth often day and night. Snores at night. He’s under a developmental paediatrician (he is autistic but otherwise no health issues) and she’s arranged for an overnight o2 monitor test because of this.

His twin brother had enlarged adenoids as a baby until he was about 2, it gradually got bigger as he grew but when he was little he sounded like a warthog all the time. DT1 is nothing like that - in the day his breathing isn’t noisy and he rarely coughs. But then at night he’s coughing again.

I’ve heard asthma can cause night coughs but he’s never wheezy, so surely it can’t be that?

Going to make a GP appointment to discuss. Annoyingly when we saw the paed a few weeks ago and I mentioned the mouth breathing she said she could refer him to ENT but I said I didn’t think that was necessary (she agreed). Now wondering if I should be more concerned or whether I should wait for the o2 test results.

He can’t tell me what’s wrong and doesn’t demonstrate much awareness of pain or anything which makes it harder to gauge things.

Night cough, with or without wheeze, definitely consider asthma. Make sure to tell GP and paeds consultant all the symptoms, make a list. Good luck.

Thank you. I feel really stupid for not bringing it up but we really have had so many bugs and I’ve just been putting it down to that. Last night was horrendous though. He hasn’t coughed once since he woke up this morning!

How would they diagnose asthma if it is that? He wouldn’t be able to blow into a peak flow meter or anything and I have no idea how I’d get him to use an inhaler / spacer!

He does have eczema although it’s much more under control since we cut out dairy, he still gets flare ups now.

They would take a full history, listen to his chest, check his oxygen levels.
eczema and milk intolerance are linked to asthma, so be sure to make a full list of symptoms and tell the doctors.

I thought asthma. Ds1 had productive asthma and is also autistic. Although I don't think they're connected.
