Best friends BiG Birthday, how much to spend?

Just want some perspective please!
It's a very good friends milestone birthday soon.
First off, I do not think she is hinting at me to buy it for her, more of a wistful "I covet this item but I can't justify it right now" I'm sure she would be flabbergasted if I bought it for her.

The item: There's a thousand different ones, I have one and I spent about £12.99 on it, but I understand its not in the same league (not bothered, it suits my needs) She also purchased one at a lower price point but is disappointed and wants a better one, she states that she really wants a generally better quality one but the holy grail is The Coveted Brand. Someone else has one and she loves it.

The coveted item is £109.
Another highly rated range is £50-£70

I had decided to buy one of the 50-70 range but then thought
a) this is a big birthday, if I don't get this item I'll probably still spend in the region of it on other stuff.
b) obviously she might be secretly disappointed that she now can't get the coveted item.
c) do I stay away from this item all together and go back to my original idea of a few smaller gifts including beauty treatments and such.

I can afford it, she's been a truly great friend. In fact, if I hadn't had her to counsel me this last year I would be in a really bad place.
I left the website open on safari and DH saw it and had a bit of a grumble about it being too extravagant, but he knows what she means to me and it would come from my own money.
Would you think I'm a show off or something if I bought it?

Have you got a big birthday coming up, or has yours passed? I’d just be a bit concerned that she might feel she has to reciprocate.

No my next end of decade is a good few years away.

But that has crossed my mind, that she'd go mad on my next non special birthday!

As you can easily afford it and she has really helped you this year then yes I think I would buy the dearer one in your position.

If you can afford it, I would be inclined to buy the £109 item, taking everything you have said into account. It sounds like a lovely friendship.
