Last week before ML what to expect

This is my last week at work. Am I supposed to have a meeting with my boss? Check if my plans have been approved? (Requested to work from home for the last 3 months).
Bring baked goods?

How senior are you? I would expect that your manager should be managing the situation, not you as you have enough on your plate! I would expect some sort of handover to person covering you and for you to have tangible knowledge/documents to hand over detailing what your tasks/projects are with where you are at and what needs doing? Something like that? The last thing you want on ML is people trying to hassle you for information etc. TBH it's amazing how the most vital work worries/priorities fade into meaningless mush once you have a newborn at the centre of your world.

I'm fairly junior. My replacement has been hired and not as maternity cover as I'll be back to a completely different role.

Nothing to handover either. So I'm extremely confused if I should remind him that I'm leaving on Friday or just let it be.

I'm a bit confused by your post.

Do you mean you finish for maternity leave on Friday? Or that Friday is your last day in the office as you'll work from home for the last 3 months? If the latter, did they approve that or did you just request it?

Friday is my last day. The only unfinished business is to know if I've been allowed to WFH for the last 3 months of the 12 months (so in 9 months from now).

I'm currently on mat leave.
Before I broke up, I had to 'request' my maternity leave on our intranet as most of our HR services can be done online. That sounds very formal but it wasn't really, all I had to do was submit the dates I intended to leave and return and I scanned and attached my matb1 form.

A few days later I received a letter from HR which confirmed my pay and leave period and included a number to call if I had any queries. A duplicate copy was cc'd to my line manager.

It has been requested and approved. We have no HR department. Everything is dealt by my manager (the owner) and if he has any questions he asks the accountant.

For the same reason, I have no documentation. All MAT1 forms have been signed and sent to get my MA.

But it looks like I could just finish my day on Friday and leave? Obviously don't plan to. So that's why I'm asking for advice

If it's the start of your ML why are you not planning to leave? It sounds like your manager is either not on the ball, or doesn't give a toss. Don't run round doing his/her job. You've applied and had it granted.

If there's not paperwork I would definitely want a paper trail re the WFH request and decision (although I'm not sure if he has to decide straight away). But it is important to get on record that the request has been made.

He said he was still discussing with his business partner who is currently on parental leave.

Well, in that case I would be seeking a date when the decision will be known to you. Try and pin him down! You don't want to spend your leave chasing this and having to make arrangements at the last minute.
