What does this mean on medical records ??

Accessing online records but can’t get hold of anyone and the key isn’t on website

[V] before one issue

[D] before another

What do they mean ??

V and D usually refer to vomiting and diarrhoea. Would that fit?

No not at all
The [V] was before an operation

[V] procedure

The [D] was before a problem I’d gone with

[D] medical issue

But no other op or illness have this before them ?? I’m just intrigued as to what it means (plus there are huge inaccuracies ! Can’t get hold of the surgery and there is no key to these codes)

Could the [V] mean verified ? I can’t think of anything else but the [D] ??
Was hoping someone who works in a GP surgery may know !

No not diagnosed as all things listed say for example

This is listed differently
The V or D is before the issue

I will keep trying to get hold of the surgery

I don’t think it’s discussed or diagnosed and it’s not anything that massively stands out ?

The [V] was before an operation name (no other operations have this before their documentation. The [D] before an illness/condition

I just found it odd how nothing else has these letters before them and I can’t see why they’d particularly stand out ?

It's to do with the read code. I can't do links but if u search for "readcodes [D]" you should find info.
Read codes are the way information is coded into go notes

[D] seems to mean disorder or diagnosis
Must be disorder as it’s said diagnosis underneath

Can’t find much for [V]

Don't know and I've tried googling medical abbreviations.

D is unlikely to be 'diagnosis' as the medical abbreviation for that is a small square.

IIRC it’s just part of the coding system used.
D suggests ‘diagnosis’.
V suggests something that may affect health but isn’t an illness as such (eg a normal pregnancy/minor operation or something like that).
It’s been a while since I had anything to do with anything in GP though do I could be wrong.
