They are not sure when the vagina ends and cervix begins because of the surrounding tissue

That letter is very strangely written, but essentially it sounds like the person this letter is about had 4 failed attempts at a smear test before having it done under general anaesthetic. Essentially the problem during the failed smears wasn’t in finding the cervix, but in realising that what they were looking at was the cervix. That’s because it looks different to normal as due to a previous LLETZ procedure the cervix has moved close to the vaginal wall and a layer of skin has grown over it. The cervix is described as being at 12-1 o’clock, and the opening of the cervix is very small, but it’s not difficult to reach and the hope is that during future smears an anaesthetic won’t be necessary.
