Anyone know this children's book?

I remember reading (and loving) a book when I was a child but I've never been able to find it all these years later.

All I remember is there were two children, possibly a brother and sister, who ventured through a wood, a hedge or maybe a fence and ended up in a strange place. One of the characters (or maybe a dog) ended up with a thorn from a bush in their foot and it was making them ill. There was possibly a witch type figure that they had to keep away from and there was possibly magical elements but I'm not entirely sure.

I realise that's absolutely nothing to go on really but does anyone have any ideas what it could be? I think I would have been reading it around 1995-1998ish if that helps anyone out timings wise.

I can't remember a specific incident with a paw, but there's a kid Jack and a wolf and fantasy/magic

Definitely wasn't a Famous Five book. I loved them as a kid though!

Not The Talisman.

I'm racking my brains to remember any other details and I'm getting nowhere. I keep thinking of things but then not being confident that they actually happened - for example I possibly remember the girl having to leave the boy at some point as he's too ill/weak to carry on but I'm not fully convinced on that.
