Do I make a complaint

I'm currently on a long term treatment plan which leaves me immune suppressed, so need regular blood checks and consultant visits etc to make sure I'm well.
Meds normally delivered to my home and taken once a month. Recently they were not delivered chased up with hospital as they hadn't released prescription turns out I was overdue on a blood test, no letter no phone call.
Blood test arranged and done, prescription can then be released, consultant then realises they haven't seen me for nearly a year emergency appointment then arranged for a few days time.
In the meantime I come down with a cold nothing unusual but it really kicked my arse by the time my appointment come round a few days later I was struggling to breathe but went to the hospital for appointment thinking my normal checks will pick up any issues and I can get advise from consultant.
Well I never saw the consultant just a nurse who asked me how I was, I told her at this point I was struggling with a cold and my breathe she didn't say anything checked blood results all good free to go.
By this point I can barely breathe so went straight to walk in centre 5 mins away who took one look at me and hooked me up to a nebuliser. It took 3 to bring my oxygen levels up and breathing under control before I was then sent to a&e
3 nebulisers and steroids later plus a chest x Ray I'm admitted as still not happy with oxygen levels etc. Released later that night armed with steroids and anti biotics, signed off work for a week.
Couple weeks later all well and good, letter as now arrived from the hospital with the note added 'I was reluctant to be examined' which is bollocks not once did she ask too was in the office less than 5 mins.
My question (eventually) is when I speak to the consultant about this on Monday do I raise an official complaint or just make it known that I wasn't examined because the nurse didn't even ask and that I was clearly unwell but this wasn't acknowledged when I informed her.

Thanks for the reply, the visit annoyed me enough but was to unwell at the time to really give it much thought. But this letter as really annoyed me as it's also gone to my doctor and is completely untrue

I would complain. If that particular person is just being lazy and passing the buck, she could be missing vital signs in her patients

Complain formally in writing.

Also, you have the legal right to request for inaccuracies in your medical records to be corrected. If they refuse to change it then they have to put a note with it setting out what you believe to be inaccurate.

This is important. Do not let them falsify your medical records. They realise they fucked up and they're trying to cover themselves. That's even more appalling than their original incompetence and negligence. How much worse could the outcome have been if you hadn't felt confident enough in your own judgement to overrule them and go straight to the walk in centre.

Totally unacceptable.

There should be info on this on the ICO website in terms of having records corrected.

And when I say complain formally, I mean bypass PALS and go straight to their formal complaints process. Don't waste time with PALS fobbing you off and fucking around with you first.

If you only speak to the consultant about it, they may at best make sympathetic noises but they won't correct your medical records, they won't record what you've said, and nothing will be done to address the underlying problem that caused it all.

At worst, they'll hold up the letter and tell you you're wrong "because it says here you were reluctant to be examined, next time you need to let them examine you...". And then it's all neatly your fault.

It'll just happen again to somebody else.

@AnoukSpirit thanks for all the advice, I will be following this up and making a formal complaint. Will also check out that website.
