What’s the point in the NHS?

I fell over at work today. My ankle was immediately black and blue and swollen, so I hobbled off to urgent care in a taxi.

I waited for four hours, I was x-rayed and told there were no breaks and a severe sprain. The doctor told me I’ve likely torn ligaments in my ankle and will be sore for three months.

I asked what sort of follow up would be given and she all but laughed at me, saying that I could get myself a support on Amazon if I wanted. I said I couldn’t even weight bear and she said I could also pick up crutches if needed. She then said I could refer to physio but “ChatGPT would give me a plan”. I asked if I’d be scanned to find out how badly damaged it is and she said there was no chance.

What is the point? Genuinely, what are we paying for when even things like this are being directed to Amazon and A.I?! I can’t believe it.

I remember 15 years ago my mum tore ligaments in her ankle and she was scanned the same day it happened, put in a boot and on crutches for six weeks, had a course of physio and consultations with an orthopaedic surgeon to discuss a plan if it didn’t improve.

What’s going on?!

For the 45% of people who have said YANBU, I'd expect them, since they don't think there is a point in the NHS, to sign a contract opting out of all NHS treatment should they (apparently not) need it.

Yes I understand that sometimes its regular visits that's needed. Obviously it's dependent on the actual problem and treatment plan needed. Sometimes it is just home exercises that are required though.

Theres plenty that can be done for torn ligaments. This is another lie they’ve told us - that for some things we just have to put up with pain and long term consequences because “they can’t do anything”!

Almost 50 years ago I trained as a Nurse, an X ray then was costed at £7.50, imagine what it would be now

Well they’ve delayed my 80yr old father getting prompt treatment (nearly 2 yrs)for a resurgence of scc that’s after cancelling appointments after the original diagnosis and treatment ,

they also put my mum when she got sepsis after her first chemo , in a ward with people with d&v (mum was a nurse for 50 yrs)

I worked my ass off today to prevent someone dying, that's part of what the nhs is for!
treatments change, 15 years a lot of research and advances has been made by then- if rest is all you need then that's all it needs. A scan won't change the treatment course so what's the point in wasting money asking for one

You got the correct treatment but wanted to be treated as a Princess! Get over yourself; you won't die and in about 5 weeks all totally fine. I had the same injury and quite happy with my treatment. Others could be life or death whilst you have a paddy at not being prioritised.

At recent NHS consultation was told to take ibuprofen for pain. Explained GP had told me I must not. Consultant just said Ah well, GPs try to fix things, we just maintain you. Completely baffled. Spoke with GP and she was horrified adamant no ibuprofen. Yes I could have meds to try to limit damage to stomach by ibuprofen but those drugs damage other organs. Absolutely amazed at how angry GP was at treatment being offered. She is referring me again to another consultant. Bit much when NHS professionals are battling with one another. It seems no one trusts one another.. Amazed I wasn't told to check Mayo Clinic or AI.

I’m not sure a scan is routine for a minor ligament injury like this. It normally RICE - rest, ice, compression, elevation. You were offered crutches but honestly nowadays they tell you to only use the crutches for a couple of days and then to use the injured ankle as much as you can - with the aid of painkillers - to aid recovery. If in the course of time the ligament doesn’t heal and you have ongoing pain, then you can ask your Gp for a referral for a scan to see if an operation is required.

I agree.
I have excruciating pain in my shoulder for months now / and elbow, not sure what caused it, I could get it injured at work and forgot about it etc, I can't lift my arm, I can't wash, put bra, its goes into shocking spasms bringing tears in my eyes daily.
I made gp appointment phone only, with gp and asked to be investigated, she said they won't..
So i said can i have x-ray or ultrasound as impossible to lift my arm.
She hesitated and said maybe x-ray but waiting list is 12 -16 weeks and appointment will arrive by post.
I said how come as before I was sent for x-ray the same day and had digital referral.
She said that all changed.
No more digital referrals, only for accident or emergency..9 weeks on- im still waiting for the x-ray.
Going away soon abroad so will pop for x-ray there..cost few quid...

The NHS isn’t free, most of us pay a fortune in tax for it- National Insurance, every month.

OP deserved better treatment, and should complain.

I’m a physio in a reasonably senior post in the NHS.
i think the care you have received could definitely have been improved. But the pressures we are all under really is unrelenting at the moment. So sometimes it will be below par unfortunately

something I didn’t know until very recently is that the budget per head is just over £17 a year-to pay for GPs, hospital care etc.

Proper treatment - in other countries a sprain is treated seriously and taken seriously! If you’re suspected to have torn ligaments you get a scan and proper treatment, not told to walk out and go on Amazon if needed. Even 15 years ago it was different!

Get a private scan and treatment plan then.

See how much that costs and what benefit you get from it

I have family in the USA and their medical bills are eye watering. Scans alone are thousands.

Because they have ridiculous prices for their health. No any other country charges prices like that.
In Europe you can have an chest x-ray for £5, mammogram for £40 etc, full blood count £15.
These are normal affordable prices .

I honestly can’t believe what I am reading from your posts. I echo others, you are so very lucky you even have the opinion that the NHS isn’t fit for purpose. Many industries are using AI, have you considered the healthcare professional was suggesting something that they know is a good way to work with the NHS at home? I am sure if you required more healthcare it would have provided to you - as proven by the multiple posts by others.

I think this says it all. The NHS is amazing for complex and emergency conditions. Specialist surgeons are worth their weight in gold.

But what specifically is lacking? Most injuries are self-limiting and heal without complications, most ailments do the same. We cannot provide a hospital bed, full workup and a team of AHP for every bump and bruise. NICE guidelines and local pathways are based on extensive clinical evidence. Forgive us if we think people should be able to self-manage minor and moderate injuries off their own volition.
OP has an option to self-refer to a community physio. That's what they are there for! Until a physio can get involved, she should follow PRICE protocol and contact her GP who can manage pain and refer for additional investigations if no improvement was made in the time we typically expect this injury to get better.
Honestly, I think sometimes people object to the sheer mention of the fact that we expect them to do basic self-management. The amount of times I heard "but a physio just gave me exercises to do at home"...well, what do you think they should have done? Spend 10 minutes five times a day doing them with you in a clinic?

The vast, vast majority of people are not net contributors to the NHS, meaning you won’t ever get close to paying in anything like you eventually ‘use’ so on that basis alone, We get something very valuable.

DH has been a net contributor for over 30 years!

My husband suffered a perforated bowel and sepsis in July. The nhs performed life saving surgery for him. He’s led next to me right now when he very nearly wasn’t.

That’s the point of the NHS.

Other countries don't have healthcare systems like the NHS yet they still manage to have public health systems. The NHS is badly designed and that's why it has the never-ending problems it does.

Lucky you if you can afford £50 without budgeting. A lot of people can't. They would have gone without any sort of care.

Thats why the NHS is there. It is pretty shit at the moment but at least it doesnt differentiate between those that can afford it and those that can't.

No it really wasn’t. I’ve sprained mine multiple times since childhood, the most recent being last year, have never had a scan and always the same treatment advice. A couple of times had physio to aid recovery but mostly self treated along the lines you have been given. Boots do a good elastic support sock in different sizes and strengths.
