Hey,

It's my second cycle if ivf, I'm on Bemfola 225 iu and it's day 4 of my cycle. Last time I was on Gonal F 150iu. Was wondering if it will produce more eggs now.

Just wanted to ask anyones dosage, the name of their ivf medication and the egg result, home many were retrieved?

@thislittlebird I'm sorry you're having to consider cancelling your cycle. When you were on 150 gonal F your results were so good. I don't get why the clinic wouldn't focus on how your body responded last time and repeat the process than changing it to just what the research says. I'm 40, my last egg collection I was 39 and 11 months. I respond better to high doses than low doses but everyone is different. The clinic should look at that.

thislittlebird · 03/04/2023 18:42

@Hyppyb they say it’s because all the research says high dose is what will work, and probably my age (40 next month) but I’m really not convinced it’s the answer for me. I find it so strange how they don’t respond based on what they can see, and weird they want to keep yours the same when it didn’t work.

If it’s still a poor response later this week we will abandon and cut our losses, it’s ridiculous ploughing ahead with such poor numbers for no good reason.

Hyppyb · 03/04/2023 15:03

@thislittlebird so for you low dosage of gonal f produce more eggs but your clinic ignore this and keep increasing the dosage. Have you spoken to them and asked why? I was given low dosage and produced no blastocyst only an 8 cell, next cycle they were going to do the same low dosage i was like no you have to increase it, they did and I got 4 blastocyst. So you definitely should say this to the clinic.

@SH998 hope you're doing OK today. I know what you mean about hoping to be pregnant before your due date so that it takes some of the pain away. It's definitely hard. We see all these stories of women going through pregnancy loss then having rainbow babies so that's what we have to hold on to. 🙏

So I have my treatment consultation tomorrow to go over my meds should be quick, will be told to take eostrogen pills and then vaginal pessaries, take pregnancy test at home.My start date is when my period comes, so I expect it next Tuesday, then my first scan to see lining is cycle day 8 to 10 and then day 17 to 19 I have the transfer.

It feels very weird trying after a loss compared to before. Now it feels real like trying for an actual baby before it was just trying but future baby didnt feel real. I feel doubtful when I think about how we're told embryos are more likely to be abnormal than normal and they can't see from just looking under a microscope but have to be positive.

You have you're follow up next week so that's good.

I can’t remember any more but as they increased the dose each cycle, the number of oocytes decreased. The successful cycle was 75 every other day plus clomid or something similar.

Sorry to reply so late to this but I’m having this issue right now.

150 Gonal F = 15 eggs, 12 mature
150/225 Gonal F = 12 eggs, 6 mature
225 Gonal F = 4 days in I only have one follicle growing, it just seems ridiculous at this stage that they keep ploughing on with higher doses. I’ve emailed to flag this but I know they just think pump more drugs in 🙄

SH998 · 03/04/2023 09:53

@Hyppyb I'm so glad all scans/appointments went well, your going in the right direction. Hopefully an April date will be given soon. Did they indicate any sort of start dates yet?
Also even better news you will be monitored more regularly, hopefully that will put your mind at ease from the start, you really do deserve a smooth run this time :)
Well it would have been my baby's due date today! I thought I would have been OK but keep thinking of what could have been and start filling up. I said after the miscarriage as long as I'm pregnant by the due date ill manage just fine, well that hasn't happened so not great. Onwards and upwards I suppose!

@Lexi41 wow that's amazing. Thank you for sharing your story. It's definitely disheartening when you're told your embryo isn't doing great but this shows that embryos can improve and that it only takes one egg.

Yes - embryologist has said that there was fragmentation - something about as the cells split into two some cells don’t recapture and just kinda get her in the middles somewhere and can reduce chance of implantation. But it worked out the end!
the clinic sent the time lapse video of our embryo and we can see where the fragmentation happened on days 3 & 4 - all the cells kinda seemed to gather at one side but in the last day it had evened out again… and it’s so surreal to see it.

I did ivf 2021 and under responded to medication for the follicle stimulation.

i had more on my base line scan and only 2 after medication. I can’t remember exact numbers but I was in the second highest dose (out of 4 levels I think )
they had a meeting to decide if it was worth going ahead and decided to proceed (I guess being 40 with low AMH levels they didn’t think I’d do much better then that anyway…)
anyway when it came to egg collection they said one didn’t look good so it wasn’t worth to use :/
so we only had one egg!
we used ICSI method where they injected the sperm straight into egg.
over the next 5 days the embryo went downhill. They said it would likely not make it but on day 5 they called and said they’ve never seen such a remarkable change and we had a beautiful wee blastocyst. At this point back up to a grade B. So they said hurry on in and we’ll do the transfer.
anyway I didn’t really have high hopes, we had already been told we would need at least two cycles and this was only our first try.
my consultant said 1 good egg is better than 20 terrible ones, but of course couldn’t make any promise it would work. We had already been told our chances were so low.
Well that little B egg is currently sleeping on my lap. :)
wish you all luck. Hope it works out for you all.

Hyppyb · 30/03/2023 11:36

@SH998 so had my follow up phone consultation appointment. It went well. The nurse consultant was like they're positive in me getting pregnant again, they understand I've been through a tough time and they're here to support me. She went over my embryos said they were good that 3BB is slightly better and that will be put inside me and said its a 40% chance success which I read its a 42.3% of pregnancy and 50.3 chance of a live birth. She gave me the option between the natural vs medicated FET. As a clinic they recommend medicated. Natural cycles are more likely cancelled. I had a medicated before, the hormones were not great but i want the best results.

Next step is waiting for the prescription in the post, arranging a treatment phone consultation to go over the meds and go ahead with the FET cycle in April.

@SH998 **THS blood test is for thyroid activity not fibroids.

OP posts:

@SH998 At the stage of an embryo transfer, my understanding from a doctor is the most likely reason for the implantation failure isn't you or your womb, it's the actual embryo. Of course there are exceptions but the part of the point of the scans is checking your womb lining and blood tests for your hormones that the embryo will be welcomed, again making it unlikely that the failure is your body.

Good graded embryos get their grades superficially, a beauty contest, ìts like saying a symmetrical juicy looking apple will taste better or last longer than a less attractive apple. No one knows just by looking. But the chances are theyre more likely to implant if better graded. Embryos usually don't implant because chromosomally and or genetically there's something wrong so they arrest and they cant tell by just looking. So don't worry too much if they don't find a problem with you. Grade Bs have around a 50% chance of not implanting.

I've only ever had one blastocyst transfer and got pregnant. My prior transfer was just an 8 cell embryo that probably didn't make it to blastocyst and arrested inside me. Yeah it's hard when you have all your hopes up and then to get a negative. I've been there 5 times with IUI also. It's a tough process.

When you have an etopic pregnancy the NHS tell you to call the early pregnancy unit to make an appointment for scans and blood tests to start monitoring hcg and the embryo the moment you get the positive on a test to catch an etopic pregnancy early. Usually your first nhs scan is 12 weeks. I would still have the 6 week viability scan with the ivf clinic not that I would need to with the nhs scanning early.

So had my scan, nothings really changed, they saw the 3 lines in my womb lining which is good. I had a TSH blood test for fibroid will find out in the follow up on Thursday. They did my BMI which is annoying but I think it was OK. I've gained weight from stress but nothing too bad. The doc I saw was the one who did the collection and transfer. I bumped into one of the nurses who was present at the transfer, she was sweet.

Overall went OK. Hope the doc on Thursday gives the green light to proceed.

@Hyppyb I'm really happy to have 3 good embabies waiting, although the 2 implanted last month were both better quality and failed, its so hard not to be disheartened and positive about it all when you just keep getting knocked back time after time. I really have to do my best to keep positive :)
I'm hoping the bloods pick up something little, to keep hearing its unexplained and everything looks fine is annoying when there is clearly an issue here. I think I will feel better going into this round knowing more investigations have been done. The 12th April will be hear before I know it.
As you say, on the plus side, I did get pregnant, I just want that feeling again.
You should be extremely excited about your next time, there is nothing going against you for it not to happen and the baby must have been strong to hang on for so long. When you say you will be monitored asap on your next pregnancy, will you be offered scans before the viability scan to see if baby's in the correct location etc?
How did todays scan go? Wishing you all the best, you deserve some good news :) x

Hyppyb · 24/03/2023 19:28

@SH998 Grade B's are really good. 😊 The majority of pregnancies are embryos that were graded as B. Grades A are classed as excellent but the percentage of embryos graded as such is a lot less. Some embryologist see Grades A as perfection so grade them rarely. So you should feel good that you have some good embabies there. Two of my embryos are classed as poor quality so I was a bit down about it but there's still a 33% chance of pregnancy, also grading is subjective, also a grade C may turn into a grade A in the womb.

If there is something stopping you from implanting I hope it's found. Although you have got pregnant before so that's really positive and implantation is the hardest part of conception, there's like a 50 to 70% chance of implantation not happening. Usually 70%.Yeah when you've been so careful it is so very annoying, we've just got to keep trying. 12th April isn't far. Will be about the time I start my meds for my transfer.

I feel partially excited that I'm trying again and could get pregnant with my rainbow baby but then of course I'm like I dont want to go through another 14 weeks pregnancy loss, technically I was pregnant for over 4 months that's how long the hcg was present. Makes me think the baby was strong but implantation was in the wrong place. I know I will be monitored by the nhs ASAP if I get pregnant. Still hard though. In the pit of my stomach but the thing that gets me through it is InfertilityTV said 69% of women who tried to conceive within 3 months of a miscarriage conceived and that was significantly higher than those who waited. His advice is try to conceive once you get your period and that's what I'm doing.

So yeah my scan should go OK. I'm going on the day I'm ovulating coincidentally. It's on Tuesday and then a consultation on Thursday. I would have thought if there were any problems I'd find out at the scan.

Will share how I get on.

@Hyppyb the bloods I had done this time were completely different from the initial STD, hormones checks. I've had to google them all as they mean nothing to me lol, blood works were for co-agulation, lupus, anti cardio lipids, anti B12, factor V, pro-thrombin. I'm hoping something little and easily treatable shows up, ill better doin the next transfer having taken slightly different steps. My follow up call to discuss results is 12th April.
Also, I've spoken to the embryologist and said the frozen's were all 5 day blastocysts grading at 5B, 4B and 3B. She said they were good quality.
I've learnt over the last few months that IVF has a mind of its own, one cycle will completely vary from another, body reacts differently etc. When you've done everything correct and are adamant its worked/should work and then it doesn't, its a kick in the teeth.
I bet your counting down the days until your scan date, the wait is an absolute killer. Please make sure you keep me updated, I have everything crossed for you :) x

Hyppyb · 21/03/2023 14:53

@SH998 hopefully when you speak to the consultant they're helpful. General blood tests I had was only for TSH and STDs. The main expensive clinic they took loads of bloods through IUIs and one IVF to check my hormones that they showed I was ready for the treatment and i never got pregnant but my last ivf was with their cheaper sister clinic who did no blood tests and I got pregnant! 🤣My prior treatments I was careful of my diet didn't get pregnant whilst last time I accidently drank coffee seconds after the transfer and got pregnant. 🤣

When I had the transfer when I didn't get pregnant they had me take 1 asprin at night to help with implantation. Then the eostrgen pills 3 times a day and progesterone 3 times a day and that was unsuccessful but I think that was the embryo that was only a day 3 probably arrested soon after. So yeah hopefully there is something that can help you.

I would recommend you subscribe to Infertility TV, on YouTube, short simple fun videos on all of this and they look at studies and comparisons. They look at embryos, your womb, embryo gradings, diet, etc

My blastocyst were 4BB (got pregnant) then 3BB, 4CB and 4CB.

When I first started trying I was positive and believed I'd get pregnant and I didn't, usually the more you try the more disheartened you get. So I'm not really going to get hung up on my emotional state. I don't have the time. If I was 25 then yes can take a long emotional break.

I'm still with the sister clinic. They changed their terms recently 37 and under changed to 39 and under, just before my 40th birthday they let me have a round and that's when I got pregnant. Crazy. This fertility stuff isn't straight forward.

OP posts:
SH998 · 21/03/2023 11:51

@Hyppyb I pushed for further bloods in the hope that something small but treatable will be picked up. They have also agreed to up progesterone and also add blood thinners into the mix if he feels this needs to happen. I didn't want to continue again on the same basis as before in fear ill lose the 3 embryos we have. Ill be asking of gradings when I have a follow up, he didn't have this to hand but said he had spoken to the embryologist before our call and they said all 3 were really good quality. Do you mind me asking what has the grades been on all yours?
I don't think we will ever be ready to go again as the worry and uncertainty of IVF never leaves you, whether your pregnant or not. You have to proceed when your ready and only you know when that is. I personally want to go again as soon as my body is capable, my head will never be ready lol.
As for 'emotional state stopping pregnancy', when I got pregnant, I was in a hell of a mess, between the stress of the IVF, unsupportive partner and generally just being in a bad place myself we managed it, yet the last transfer when we were both far more happier and stress-free it didn't work, that was a complete killer to me because if it was going to work it should have been that time! I think once your body is ready it will happen regardless of your frame of mind.
I'm so pleased you have finally got here, its been a long road but I'm sure this will be your time :) x

Hyppyb · 20/03/2023 16:26

@SH998 wonder why your clinic are doing follow up blood tests. A follow up with your doctor will be interesting. When mine failed before I got pregnant they were like chromosomal issues with the embryo that's it. It's like there go to. Where you had two embryos if I was you I would push to know the gradings of all the embryos. Like what are they classing as good quality. If its the lowest 4CC then you had 30% to 40% chance per embro. Still pretty good and better than natural conception.

If all goes well I will have my transfer next cycle that works out like 6 weeks from the end of my miscarriage bleeding. So pretty soon. I feel bad and bit scared, I don't want to go through another etopic pregnancy it's life threatening and the recovery is long. But then I hear trying to conceive within 3 months of a miscarriage you're more likely to be successful and I don't want to wait around doing nothing.

A friend said I should wait to feel better but I feel like if I wait I'm never going to feel the version of better she's referring to. 3 months bleeding was a long time I came to terms with what happened during it. I remember the 3 weeks of being pregnant and it feeling great and I think that is overriding everything.

I don't know how much i believe in the emotional state stopping pregnancy. If its stress stopping ovulation yes I get that but if you're having regular cycles, not getting pregnant, people are like it must be your emotions or mind, that doesn't explain why women conceive under traumatic experiences or more are likely straight after a miscarriage than waiting. I think human fertility (unlike many other animals) isn't the best, so many factors involved so I'm not going to let how I feel stop me or wait until I feel "better".

So next Tuesday I'm hoping my scan goes well. Then another transfer it's my 3rd but my 2 blastocyst. Should be the 3BB.

SH998 · 20/03/2023 10:05

@Hyppyb how are you feeling? I bet it feels good to have a little normality back to you life :) I bet you've never been so glad to see a period in your life lol, that's great news and not long a wait until the end of the month either.
How you feeling mentally about it all?
I have follow up bloods this week with a consultant appointment in a few weeks to discuss, no actually transfer date ahs been given yet.

Hyppyb · 17/03/2023 19:23

@SH998 hey, how was the last week of your two week wait? Was is negative? If so have you spoken to your clinic?

So today my first period properly started. I called my clinic and have arrange a baseline scan on the 28th will have a follow up consultation on the 30th and if all good, a treatment consultation to talk meds and then a frozen transfer next cycle.

OP posts:
Hyppyb · 06/03/2023 21:42

@SH998 I'm sorry. A negative when you're trying is always hard. They would say wait the whole 14 days to be sure and test then. I think youre 9 days now? It still could be positive. I do know how you feel, if it's not positive by 7 days then most women write it off. At least you have you embies or embabies. One of those could be the one. Can you try again soon? Like period after next?

So I haven't contacted the clinic yet because I plan on waiting to make sure my body is fully healed by waiting until my second cycle, so 2 periods from miscarriage to have the transfer. I'm waiting until my first period then I will call the clinic for a follow up scan. Yesterday i had wind pain in my left hip, nothing bad but that's the early symptom i had with my etopic pregnancy. So don't want to go too soon and they say there's a problem. I only stopped having pain around 3 weeks ago.

If my period goes back to normal time wise then I should be having a transfer mid April.

Have you been given a transfer date yet? I bet you cant wait to start again, its certainly been a long 3 months for you hasnt it x

Hyppyb · 02/03/2023 15:26

@SH998 I am so chuffed for you. You're in a really good position. It's just nice to have back up of other embryos, I found that really stopped me from being tense through the transfer and after.

Some of your results are above average like mature eggs the average is 90% will fertilise, all yours did and all 8 making it to day 3. You go 5 blasts, I said you should get 5 from your scans.

I'm surprised your clinic doesn't give you the grades. I feel like they're being paid a lot of money and should be transparent. What their definition of high quality might not be the same as their patients or another clinic. I get a written report with the gradings at each step. Mine rang me at day 3 telling me the grades. Saying all that yours are clearly 3CC or above. CC is the lowest grade they should be transferring embryos in to your body. I read someone say they had C grading and was told by their clinic it was high quality when I've read everywhere C is poor quality.

So I woke up everyday at 4am needing to pee but I did that every 2ww. 😄The way I knew I was pregnant was day 2 and 3 post transfer I had cramping, implantation cramping but not everyone gets that. I had no other symptoms. I had more symptoms when I was negative prior. Day 4 post transfer mine was negative so don't worry many women get those results and just use it now to see your trigger is out of your system. I got a positive on day 6 and it was darker day 7 and I was like I'm soooo pregnant. So youre not out the game at all. 14 days after is out the game. The only pregnancy test I rely on is First Response the cheapies strip early on are rubbish.

I've stopped bleeding! Quite poetic. All of the winter I bled 3 months and spring I stopped. I've seen signs of ovulation so that's good. My body did go through a lot. I honestly don't want to go through that again. 12 weeks is a long time.

I am feeling positive. Next month I should be having a transfer with my 3BB embryo. Just going to chill for now as my body gets back to normal with ovulating and periods.

@Sunshine2243 I think I read on the nhs website page on ivf what everyone else has already said, if youre older so 35 plus they would allow you to transfer 2 embryos as the idea is older women have more abnormal embryos so the likelihood is only one will be successful. Younger women are advised to transfer one at a time but I know someone who at 29 had ivf and transferred 2 and had twin boys.

OP posts:
SH998 · 02/03/2023 13:03

@Sunshine2243 hi and welcome :)
I was offered a transfer of 2 but I think that was due to my age (39), they don’t normally recommend more than 1 but I also think different health boards advise different things to different people x
