I’m on my first day back at work after 10 days annual leave.

I was walking down the road this morning when I rolled my ankle and fell, I have horrific grazes on my knee and elbow, and my ankle is swelling up hugely and is painful even at rest.

AIBU to turn up, explain what’s happened and go home? When I’m cleaned up I could probably work but I’m in so much pain at the moment I feel sick, I have two fresh grazes that are about to bleed through my clothes and I don’t know whether the adrenaline carried me to the bus or if I can walk.

You could also have a complication of the fracture which is Complex Regional Pain Syndrome. Do the skin and colour look normal? Is the ankle hot? And the pain level from 1-10.?

I have been having the same problems as you with my ankle. I went to a&e over 2 weeks ago. They said it wasn’t broken at the time but the pain has got worse. I tried to see my GP but she cancelled my appointment because she was sick herself and went back to a&e last night, one dr complained that I was there and said I should have seen my gp but I’m sure she would have sent me for scans etc. I now have a splint as I have tendinitis. And new painkillers because the others were making me vomit.

I hope urgent care can help you with this - I think you've had poor care until now and have been very stoic in the face of it all. Please do try to rest if that's what is advised - you need your leg more than you need to be in work holding work things together in what sounds like an understaffed and stressful environment

From what you say it sounds that there is something very unstable there. As I said, we put our patient on the bike the day after his op with no problem.
Go back to the GP or specialist and insist on getting a proper diagnosis.

Christ!! I can’t imagine doing that right now!

I’m going to urgent treatment tomorrow. I can’t hack it anymore, it’s back as swollen as it was when I did it (if not more) and I’m able to walk even less

PS You could use a home bike. One of our patients was a champion biker. He broke his ankle, was operated on and our surgeon told us to put him on the bike the next day - with NO resistance, just free pedalling.

Still, in either case you should be wearing a cast for 6-8 weeks as I said before, to prevent future instability. Doesn[t have to be a plaster, a removable one will do.

Not that I can see via the app. I’m not sure how I’d find it?

I had an xray, he told me face to face it was an avulsion fracture but in the discharge summary he’s written sprain! I’ve requested my notes through our hospital app (I only just found out you can do that!) so hopefully there’s some clarity in there. If appropriate I might then go to my gp again and see if they can order some more imaging, but I’d imagine that’ll be so slow that they won’t be able to see anything by the time it comes around!

surely this would be something you’d ask your GP about? They’ll even be able to bring up the scan and report

Not yet, but I was under the impression you need to report everything you’ve seen a doctor for to your travel insurance!

Thankfully the various lotions I have from infected bug bites on holiday have helped my knee and it’s scabbed and over. It’s still very, very painful and there’s a lot of numbness, but I’m just accepting that as what it is.

My ankle is still causing me a lot of bother. I’m still really bruised and swollen, trying to elevate as much as possible but I also can’t just sit about forever. I’m still waiting for the radiology report to come through onto my NHS app and I’m hoping that that might shed some light onto the situation. If not, I’m going to request my notes from my visit to a&e and see if I can uncover what I’ve actually done. I feel very frustrated because it’s been another few days of doing the bare minimum, but nothing is improving. I feel disgusting - bloated, tired and sick, which is all down to not moving as much as I want. I miss the gym and hiking but I’m in no shape to do that at allb

@iminpain glad you have an opportunity to rest and recover. Sprains are nasty. If you can wear compression please do. I sprained my left ankle 15 years ago and it is still bigger than my right ankle.
Also sympathise with your A+E visit. I went with pain in my upper abdomen- told them everything that had gone on. Waited 6 hours then got discharged- still in pain. Got a postal letter 2 weeks later saying "I was the consultant in a+e when you were in and on reviewing your x-rays you have 2 non displaced fractured ribs"

I’ve sent this email to PALS:

I went to A&E on 9 September following an ankle injury and had an X-ray taken.
The clinician who assessed me told me face-to-face that the X-ray showed a minor/stable avulsion fracture. I was told to treat it similarly to a severe ankle sprain, with compression, crutches as required for pain, and weight-bearing/walking as tolerated in sensible footwear. I wasn’t given a boot or splint.
However, my discharge documentation records the diagnosis as “sprain” rather than an avulsion fracture.
I’m not complaining about the care I received — I’m just confused by the discrepancy and want to make sure I understand what was actually found.
I’ve contacted PALS to ask:

  • Whether the X-ray did show an avulsion fracture.
  • Whether my records should reflect this rather than simply “sprain”.
  • Whether the X-ray has had/will have a formal radiology review.
  • Whether I need any follow-up, such as fracture clinic/orthopaedics, repeat imaging or physiotherapy, or whether I just continue with the advice I was given.
I’m particularly keen to clarify this because I have upcoming travel booked and need to give my travel insurer accurate information about the diagnosis and any follow-up required.

(Obviously it had personal info in), I really can’t be bothered with the complaint as I don’t think they need to deal with that on top of their day to day jobs!!

@iminpain My go to for nasty grazes is honey (actually it is for most things unless there is a suspected fungal infection).

Slap on some honey - now ideally its clean, new, Manuka honey but frankly, as long as its not got crumbs and butter in it, it'll do just fine - stick that on some kitchen roll if thats all you have, stick that on the wound and tape it down using whatever you've got.

Its antibiotic, anti microbial, it will pull out any crud that is in there and help things heal from the inside out - you really don't want a graze like that to scab over as the scab will pull and yank each time you move the joint, making the scarring on the skin worse in the long run.

Change the dressing once or twice a day, particularly if you've had to DIY it with kitchen roll as air to it is good but also as it pulls out liquid, it will get pretty soggy too (if you're using proper dressings you don't need to change it more than once a day).

Gosh I can’t stomach that! I’ve got some savlon faster healing gel on, which I’m hoping will do the job!

Yes, this is the other thing - many workplaces don't even appreciate it if you struggle in when you should really be at home recovering. It's a mug's game.

I agree with this. Ime the more you do the more a certain type of manager will just see you as a mug who will put up with anything, so you end up as the person with all the shitty shifts/jobs etc.

It will be very sore for at least the first couple of weeks. Take care, only do what you feel is necessary. Don’t overdo it just because you think you should be - listen to your body.

Something to be aware of with some injuries, those who are saying just get on with things, and that is they can cause long term chronic health conditions, which is what happened to my dd. She has CRPS and all that started with a sprain which she was told to walk on after a day or so rest. It doesn't happen to most people, I broke my ankle after slipping over in our garden, but it's something to be aware of.

i feel better in myself but still in a lot of pain. Hopefully a good weekend in bed horizontal will help

Go to work and seek painkillers from first aid cupboard. Do your best to work for the day.
You need to reconnect. If you think you need an xray etc... organise from work and take the rest of day off.
But I am stoic and born last century.

I was born in the middle of last century and I'm telling you there's stoic and there's also stupid. You have obviously never sprained your ankle badly enough to pull a fragment of bone loose. A bad sprain is often more painful than a broken bone. When I sprained my ankle badly I could barely hobble, let alone walk well enough to go outside. Work does not take precedence over health, or at least it shouldn't do.
