To accept weaker orgasms

Hi I am 45 and have always had a decent libido and good orgasms. I suspect I’m in the perimenopause and the last year my orgasms are just so weak. Is this something I have to accept as part of aging? I’ve tried pelvic floor and used a Kelgel machine for a year (improved bladder function but hasn’t increased them). It’s making sex not worth the effort. Is this normal and something I just have to accept?

@Lennon80 get yourself on Vaginal Atrophy group on facebook. This subject has come up numerous times and there is good advice there.
You can take all the vitamins you want but at the end of the day they will not solve the problem. Your clitoris will shrink after menopause and a lot of women loose the feeling at orgasm. Search the facebook site, so many in your position.

Ashwagandha (That I take to help me sleep) has increased the intensity and the availability (iyswim) of my orgasms.

I had my tumour tested and it was oestrogen receptive so oncology have always said probably only progesterone not o easteogen

My breast tumour was estrogen fed…..but the amount of estrogen you get in hormone gel/cream in a year is the equivalent of around 2 daily doses of hrt….and it’s not systemic

I totally get it. I'm late 40's, peri, and recently started on sertraline for severe anxiety, as well as HRT.
I have to work so hard to climax at all, and when I do, it's frankly pathetic. I used to have wonderful orgasms.

Antidepressants including sertraline are well known for this alas.

It is indeed a hormone cream. I never said it wasn’t. Are you saying that OP can’t use it due to her background? Unlikely to be the case for topical HRT, which can often still be used by women with history of cancer (although I don’t remember whether OP mentioned such.) Obviously under discussion with GP but as per post upthread from @doublec not all cancers are hormone fed.

Yes, it is a hormone, but it's incredibly low dose and localised. Even someone like me with a hormone-fed breast cancer can use it. As mentioned in an earlier post, ovarian cancer is not hormone feed, so using topical oestrogen is fine - had this very conversation a few times with Gynae/oncology in advance of having a Salpingo Oophorectomy

GPs aren't all au fait with a lot of HRT as it is, and as Vagifem and other topical oestrogen creams are an add-on (so to speak), it's probably best OP starts with her OAFU/MDT about getting it prescribed as they can write directly to her GP. (Indeed, when I mentioned to my GP I was having issues with atrophy and had start using it, they had no idea what Vagifem was, nor atrophy, confusing the latter with vaginismus. Sigh,

Honestly! You lot! 70 is my year of birth so no, am not an old hag who sailed through menopause, am right in the midst of it. What is wrong with thinking there is more to sex than just having a powerful orgasm at the end of it? I mean, orgasms are great but if something happened so I never had another one, I wouldn't think sex wasn't worth the effort anymore. I like the closeness and the whole act. The orgasm is an added bonus. Am I easy pleased or something?

Yes. It's 2024, women are allowed to want an orgasm.

My orgasms were a bit crap for a couple of years in my mid forties. Then I took iron tablets for anaemia and boom! they came back full force. Might be worth a blood test.

Jemimapuddleduk · 09/11/2024 23:03

I’m with you OP, I would not be bothered with sex if I didn’t get a decent orgasm out of it. The Yes o yes clitoral stimulant is pretty good. I also use the lube and moisturiser in the same brand. The joys of peri.

Pussycat22 · 09/11/2024 23:03

GoingRoundThatBlockAgain , possibly but it is HORMONE cream.

That'll be the Sertraline. Have always had the BEST orgasms... however, could barely climax at all once I started on that.

Taking the meds wasn't negotiable but nor were crap orgasms going to become a thing. So I skipped a dose for one day... and, that night, well, let's just say, it worked.
So now, when I think DTD is on the cards, I'll skip that dose. Works every time.

doublec · 09/11/2024 22:58

@JaceLancs A lot of people don't get on with the patches due to the adhesive. The fact that you were having issues with sore breasts and itching skin and anxiety is symbiotic that you weren't on the correct does for you anyway. HRT is an art, and most who prescribe it don't have the know how.

There's no reason why you should try vaginal oestrogen. I'm also post menopause. Vaginal oestrogen stops atrophy which is incredibly common in post-menopause.

Loonaandalf · 09/11/2024 22:57

Could you go see a gynaecologist, you may need to pay. I’d imagine an nhs gp will just fob you off like they do with most things. Other things to consider.. sex toys, sensation enhancing gels, less masturbation/ less sex and see if that helps with intensity when you do have an orgasm.

doublec · 09/11/2024 22:55

@InsiderBetty I don't think there's any reason why not. Talk to your GP or else a pharmacist - Vagifem is available over the counter as Gina. For me, it's been a magic bullet that I wish I'd have had access to years ago.

JaceLancs · 09/11/2024 22:55

I’m post menopause and had a hysterectomy when I was 35, tried HRT patches and didn’t get on with them due to itching skin, sore boobs and feelings of anxiety
My orgasms whilst still pleasant don’t feel as deep - any idea whether a cream would help?

InsiderBetty · 09/11/2024 22:47

princesspadam · 09/11/2024 22:06

I'm on HRT
The vaginal pessaries and testosterone has given me my orgasms back and I am super happy with this!!!!!
It's been a game changer
Also on sertraline

Can someone who isn't peri yet use the vaginal pessaries does anyone know?

ABirdsEyeView · 09/11/2024 22:40

You might want to look into testosterone as well. Superdrug do it if you can't get GP to prescribe (you'll need to get your levels tested first though).
Menopause makes our testosterone drop as well as our oestrogen.

I’ve had ovarian cancer in the past so hrt is something I am going to need to discuss with oncology. The cream sounds great!!!

Ovarian cancer isn't hormone fed, so HRT isn't an issue per se. Also, topical oestrogen isn't always considered as standard HRT and not routinely offered. But it should be, it's miraculous stuff. (Indeed, my orgasm are stronger since taking it).

As ovarian cancer isn't fed by hormones, and I know this because I've recently had this conversation with a gynaecologist at The Royal London in advance of having my ovaries and fallopian tubes taken out. I had hormone fed breast cancer and a double mastectomy. Due to BCRA mutations and family history, I am at a huge risk of ovarian cancer. While I'm not allowed HRT, with the blessing of both my oncologist and gynae, I use vaginal oestrogen in the form of a pessary. (In fact, I forgot to mention I was using it and the gynae recommended it to me). It's an incredibly low dose, does not enter the bloodstream and is safe. Just ask your MDT or cancer nurse about it. I use Vagifem because it's the easiest to use.

Talk to your team. There's really no reason why you cannot use it.

Hoglet70 · 09/11/2024 20:12

If the only reason for having sex is the strength of the orgasm at the end maybe you're having sex with the wrong person? Not meaning that as a snarky comment, even though it sounds like one.

What's the other reason meant to be?

Surely sex is to experience orgasms and pleasure.

No man would bother if they didn't orgasm, so why should women?

paddyclampster · 09/11/2024 22:32

I haven't noticed a decline but I am on HRT!

I think sertraline can do this.
