Holes in my medical notes - how do I sort it out?

I have moved area several times and I accept that transferring data is hard

But I'm trying to claim PIP and the letter the gp has sent is missing huge bits of information and diagnoses

I have requested this by an e consult and got a letter missing several surgeries, conditions and even current medications

What's the best way to address this?

You definitely need to go into the surgery . However, I have to tell you that between uni and moving back home, DSs medical notes got lost with all the most important information about his health and they have never been retrieved.

I had this without moving house. My notes were missing chunks of important health information and numerous dates were incorrect. I only found out when I requested a copy. Unbelievably they sent me someone else’s notes but thats another story.
GP practice had changed computer systems so the inaccuracies in my notes were caused by this. I complained to the practice manager and eventually the senior partner called me and we went through my notes correcting them. It was a long and stressful process and I was horrified that vital information had been missed out and I might never have known.

Filled in a second econsult saying things were missing and included a list of several things, medications, conditions and surgeries that were missing; explaining that i was filling in the pip form but while doing that i realised my notes werent complete and i was concerned about all the gaps

and had a snippy text message back saying a tonsillectomy when I was 21 won't affect my claim for PIP

Well I know that, but my hypermobility, inconvenience (not fixed by 2 bladder repairs) prolapse and rectocele plus multiple allergies will affect pip so why look at the one things that doesn't?

I felt dismissed and my feelings negated

So I rang, apologised to reception and proceeded to cry all over the place while explaining

So I'm getting an appt with a social prescriber to do the form and making a complaint about the text

But my notes are still not complete

I’ve never used my GP notes for my PIP claims, I use my hospital notes. It obviously does take longer to get them, but they’re the ones that show everything I’ve had done.

I’m going through a review atm and sent in 57 pieces of evidence I already had, but there was 3 month wait for my notes because I “complicated” apparently. I’m in the process of printing off 294 pieces of information that cover me since my last award.

Has the new surgery received all your notes yet? Nowadays, it’s all done electronically (gp2gp transfer) so should come across automatically.

Doctors records usually consist of several sections. There’s the main records, and also the document files. Hospital letters get stored into the document files, but then someone has to physically read the letter and input the info (‘summarising’) onto your records. If this hadn’t been done, (or done well) then the doctor could easily overlook stuff. He’s unlikely to look over all the letters when writing a report, only your main record.

Isnt PIP dependent on your degree of functional day to day disability not your medical history?

Phone the hospital. I think it's data protection who deal with it but I just asked to speak to the person who could give me a copy of my notes. Mine were emailed and to me but in the past they were posted.

Yes but they do say they want evidence for PIP, so I assumed a basic diagnosis was necessary

Irrespective of PIP I need to know the surgery have the basics right - currently they aren't even aware I have a mesh bladder repair or that I'm hypermobile
