Kitchen/Diner/Living Space & Seperate Bathroom/Utility Room

Good Evening,

I am in the process of extending our current separate kitchen and dining into an open kitchen/diner with additional living space for a L Shape sofa and TV while also having a separate bathroom and utility room.

We are at the space planning stage and have come up with a few challenges:
To maximise the space we would like to extend the Rear and Side of our current diner, which from what I have read does not need planning permission as the length of our rear extension is less than 3m and side extension is less than half of the house (as show in the picture).

Firstly is there anyway we could easily extend 'Wall D' to the length of the proposed rear extension so we have a square like shape without planning permission?
If we do need planning permission, how do we go about getting a design to utilise all our requirements?

Is there anyone you could recommend or know of any companies that can assist with planning with our proposed space structurally as well as assisting us with how to design all of our requirements?
