Well I tried to do it but it's not mobile friendly at all. I couldn't check any of the square boxes. Typing anything was a pig and came out a garbled mess. I submitted what I could, but have no idea if went through or not as it just came up back at the start of the empty survey.

Oh my word the buttons though! The circle radio buttons are perfect but the square ones each took 10 to 20 clicks to activate - even when I zoomed in. I nearly gave up. (I'm on an android phone)
