Crb what evidence needed?

can anyone tell me what docs are needed as proof.

Off top of my head:
Proof of address (less than 3 months old)
Proof of identity (preferably with photo)
At least one document that corroberates at least one of last two items.

Star will confirm with ds who is senior advisor for CRBs in our county but when I needed one they needed proof of identity and address so passport, driving licence or long birth certificate plus utility bills.
