Referral forms for ASD assessment

Next week we have a meeting at school with the health visitor, SENco and dd's teacher to fill in the referral forms for the ASD assessment. I'm so worried I will miss things off - i'm trying to make a list and i'm going to take a notebook with me as I don't have the best memory especially when its a full on situation.

What sort of things should I include on my list - is it just behaviour concerns/traits?

I found using the list of issues in Table 2, Appendix C of NICE CG128 (2011) "Autism in under 19s: recognition, referral and diagnosis" really helpful for prompting me to write a list of concerns or examples of odd behaviour. Pay attention to behaviours before 3 years and don't just cover now. For example include issues about when/how speech developed and gesturing/eye contact as time has passed - particularly pointing to share attention with you (as opposed to pointing to get something that they want). Don't forget motor problems (eg cutlery/handwriting) and sensory issues. Good luck
