DS wont sleep in his pram or actually any time in the day either

My DS, 5 m.o is a perfect sleeper at night, always has been, 8pm to 5am, but im beginning to wonder if this affects daytime sleeping.
It is obvious he is tired but he will just scream, even in his pram, long walks dont even calm him. We've tried the dummy, which he accepts at night, but spits out in the day. The only way he falls asleep is for me to bf him but he then feeds for about 45 mins and will wake up, also i always feed him before we leave the house but he will still perform! Any suggestions?
