supplementary application forms

this is just a little follow on from the appeals threads for my curiosity.
If all admissions are up to LEA and generally based on distance why does virtually every school I applied for have a supplementary application form?

If they are faith schools, they will have a supplementary application form to establish which of their admission criteria your child falls into. If you take a look at the school's admissions policy it will give details of these criteria and how they prioritise children applying to their school. For example, a catholic school may ask on their supplementary form if your child is baptised catholic and will ask you to provide a copy of their baptismal certificate to confirm this information.
