Ectopic or threatened miscarriage

Hi all, I am in a bit of a conundrum and would like come advice.
I had a mmc in Feb followed by an MVA procedure to remove the fetus. Got pregnant again end April (OT positive on 27th April) and started spotting at around 5-6 weeks (23rd May). Went to EPU at the local hospital where they did a scan on which they couldn't see anything (in uterus or ovaries) and hcg blood tests 2 days apart with levels 78 (24th May) and 65 (26th May) so was diagnosed with a miscarriage. Had bleeding for ~5 days. The bleeding was like a heavy period and more reddish in colour than brown though I did not pass any big clots and settled after 2 days to a light brown discharge which also stopped after a couple of days (Sorry if TMI!) but I did not experience any pain or cramping. I did a HPT after 2 weeks on 8th June which gave a strong positive so went to the EPU again and they did blood tests after my insisting which came out to be ~1000. I am again having pregnancy symptoms like tender breasts and nausea. I also feel bloated and have some discomfort in the abdomen. Given the high hcg levels they have called me in for a scan on Friday. I called my GP and she said my symptoms dont shout ectopic as I dont have much pain. However reading on forums tells me people have varying physical discomfort during ectopic pregnancies pre-rupturing. Given the varying hcg levels does it shout ectopic? Also will Friday be too late and I should insist to be seen sooner? Or was it just a threatened miscarriage and I should wait till Friday to be seen? I am very confused and scared atm regarding ectopic and DH seems to be very positive and confused why am I feeling so anxious. Confused

Sorry you’re going through this. It all sounds like a possible ectopic to me - pain doesn’t really have too much bearing on it. Your levels at the end of May weren’t rising normally, but even if you’d only had the one test then, the doubling until now isn’t right so I don’t think it seems that you’ll have a miracle outcome unfortunately. I was trying to figure out whether this could be a new pregnancy but again the timeframe doesn’t fit.
I think you’re fine to wait until Friday, but pay attention to any changes in symptoms, and get seen sooner if you do get any pain. I’d make sure you either have someone with you or a phone close by to call someone if you suddenly feel unwell - there’s a small risk of rupture if it is an ectopic.

I’ll add - just reread and saw you are having a little discomfort - if you have any fainting spells or dizzy feeling please also get seen more urgently, as well as if the pain increases. X

Thanks! I am 100% certain this isnt a new pregnancy. Havent DTD since before I started bleeding (mid May). I checked the doubling, it seems they are doubling every 3 days. I am less concerned about a viable pregnancy more about an ectopic one.

Hi - I had an ectopic in April and my HCG levels were a little all over the place. I had a negative pregnancy test in A&E but positive test when admitted to the gynae ward the same day. HCG doubled on one occasion and grew very slowly on another. I bled the whole way through on and off (brown discharge some days, bright red blood on others).

I’d be prepared for an ectopic if I’m honest. Mine was treated with an injection, which did make me feel crappy but I’m grateful I still have my tube.

Please be alert to any dizziness, worsening pain or shoulder tip pain as this could be a sign of rupture.

Can I check what your levels were when you were treated with an injection? I have had a consistent positive HPT since last Tuesday so not really fluctuating but consistently present. But I see what you are trying to prepare for the worst but really hoping is just an incomplete MC (never thought I would say that)

They were 500 when I was injected - my pregnancy tests were always consistent and quite strong
