Day 21 Progesterone Blood Test

Hi all,


I have been TTCing now for over 2 years.


Doctors have done blood tests and found my testosterone levels were high, repeated those which then came back at normal levels. Next they ordered Day 21 Progesterone Blood tests, had these carried out early this week and have just received the results back.


According to the bloods my Progesterone levels were at 15.8 nmol/L on Day 21. Have been told that this value is below that of what they would expect for full ovulation to have occurred. Does anybody know what the normal range is that would mean ovulation occurred? Also has anybody else gone through this, and could tell me what there next steps were?


I am feeling so sad and let down. Any advice would be appreciated.

Above 30 confirms ov.

They may want a repeat of results next cycle. They may run additional tests... Is this with a fertility clinic or GP? If fertility clinic, they might look at prescribing something like Clomid. Do you have regular cycles and have you had Day 3 tests?

Day 21 bloods would only give you an accurate reading if you have perfect 28 day cycles. The test should be done 7 days before your period x

Hi all,

I had my my 21 day progesterone blood test a couple of weeks ago and they said my levels read at 17 which means I ovulated. They said they aren't going to put me on clomid yet because of this. Did they get it wrong?

My DH and I have been trying for 2 years now and finally got around to the stage where I would be prescribed clomid as I believe this is where the issue lies for me (though not confirmed - all tests came back normal). I am irregular and if I had to guess I would say I have a 33 day cycle but that's only because I have had 4 of them with the rest always being late. I went 4 months without a period at the beginning of the year.

I'm now frustrated because the doctors won't give me clomid because this time I happened to ovulate (so they say). I had day 23 test by my go before this one and it came back abnormal but no further explanation was provided as I got referred to the fertility specialist.

Should I be asking for another day 21 blood test next month? My result was definitely not over 30 and now I feel like I have to wait even longer before there is any chance of getting pregnant.

Extra info is I am 29 going on 30 and I had my tubes checked and all is good. Had an ultrasound where they said I looked like I had polycystic ovaries but again nothing else was ever said about this.

I had Day 5 tests which showed high testosterone, had then repeated they were normal and had them done again which again were high in testosterone. This is with my GP. Then I went back and they said this Day 21 test which showed 15. Just so confused.

I would ask for a repeat of day 21, that's what I am going to do because I'm so confused as to why it showed 15. Surely if I'm not ovulating at all progesterone levels would be low not half way between normal.

I had a day 21 result of 24 which my GP said was equivocal. Fertility consultant said over 30 confirms ovulation. Had another result of 40. I was prescribed Clomid as it looks like I sometimes ovulate and sometimes not. I've had 5 cycles now with no monitoring scans, had another day 21 test but then cycle was longer so timing wasn't great.

If they are using the same values with scores of 15 or 17 that does not confirm ovulation. I have regular periods and had no idea there would be a problem.

Sorry to hear about your ovulation troubles. I think clomid is what I need but not sure if GP will give that to me, think he will repeat bloods first. Hope it's okay to ask, How long have you been TTCing?

I'm also having regular periods, they come every month like clockwork so when they said levels were abnormal etc I was so surprised! I thought normal periods normal ovulation - obviously not.

Do you have a 28 day cycle OP? Progesterone level to test for ovulation is meant to be done 7 days before the start of your expected period, so if your cycle is shorter or longer than 28 days it needs to be done the same number of days before/after day 21 e.g. Day 23 of a 30 day cycle.

Your GP shouldn't be prescribing you clomid as before having this you need a test to confirm your tubes are patent and when taking it, you need monitoring with scans to check you're not over responding to it. Clomid needs to be prescribed by a gynaecologist or fertility specialist as without the appropriate monitoring it can be dangerous to take -risk of OHSS and multiple pregnancy (in high numbers, not just a risk of twins).

Thanks EarlGreyT - that's really helpful. I didn't get my blood tests done 7 days before my period unfortunately. We will just have to keep trying until something happens or they test me again.

TTC is so frustrating - feels like a never ending merry-go-round. Everyone else around me makes it look easy too!

Hi OP I have been TTC for years, consultant prescribed Clomid as last ditch attempt before IVF.
Perhaps the most useful things you can do are have tests repeated and push for a referral for fertility investigations. Print out reference values if necessary because if you are not ovulating you are falling at the first hurdle so to speak.
I did have a HSG soon after starting Clomid to see if tubes were clear.
Earl the NHS consultant prescribed 3 months worth of Clomid and contacted my GP to ask them to prescribe a higher dose. I have taken it for 5 cycles and never had any kind of monitoring. I pushed for a repeat progesterone test myself as I had no way of confirming (apart from temping, OPKs) whether ovulation was actually occurring. I'd had previous tests on day 24 of 31 day cycle. It was of course sod's law that my last cycle was 34 days so although the result was high the timing was not 7 days before period started. I had always had "peak" on Clearblue tests and had regular periods.
I know it's ideal to be offered scans to check follicles and ovulation but in practice it doesn't always happen.

Hi I need a little advice

I came in my period last month as normal, (28 day cycle) which lasted 7 days however five days later I came in AGAIN which lasted another 7 days. This has never happens before. Cutting to the chase I have my day 21 bloods which I need to book however no doctor knows when I should count day 21 from? I’m very confused weather to count from my normal period or the period I had five days later. Bare in mind my period was right on time 28 day cycle however this period arrived five days later way to early. I’m thinking day 21 of the first period? Any advice would be much appreciated , thank you.

Hi

I know this thread is old but wondering if someone had same experience as me.

I had 21 day progesterone test and results were 38 which gp mentioned anything above 30 confirms ovulation. I had a repeat one and the results was 29 however I think I mis timed it by a day or 2 earlier.

I spoke to the nurse and she mentioned even 38 is on the low side- what does this even mean? They have put me on clomid for a few cycles and then have another appt to see what the next steps are
