Breast reduction.

I'm looking for some advise. I have very large breasts 34H. And I have a lot of severe upper back, shoulder and neck pain.
I hAve been having physio for the past year and we have talked and he said there isn't anything else he can do and he is going to write in my notes that a breast reduction would be a possible next step. I am hoping to have this done on the NHS and am aware that this will be a long process. Physio have advised to start this process by seeing the gp.
However I have just found out that I am pregnant. I know that I will not be considered for surgery until after I have finished breastfeeding. But I wondered if I should book an appointment anyway? Get the ball rolling or make my gp aware that this is what I want? Or should I wait till after I have had the baby? I don't want to go in 20moths time and for the gp to think I have been fine all this time when I am not fine. Just looking for advise on how best to proceed. Thanks.
