Reflux??

First time mum here so a little unsure if baby has reflux or something else.

Baby has the following symptoms.
All of which have been worse this weekend. It has been slowing building last few weeks. We have our 6 week check Tuesday but just want a bit of back up from experienced mums if possible please.

A bit of background... baby was found to have slow growth at 32 weeks, had scans which put him on 5th centile line then at 36 week scan he went down to 2nd centile line so was induced at 37 weeks

Once born he struggled to feed so ended up on formula with me pumping (I don't seem to produce much milk though so he gets about a max of 5ml a day)

As he wasn't gaining much weight at he had several weigh in's at 5 says, 7 days, 10 days and finally at 12 days was back to birth weight. During these weigh in's we expressed a concern with his slloooooow feeding and the small amount he was taking but was feeding every 2 hrs he was found to have a bad tongue tie.

Now he has had the tongue tie sorted 14 days ago.

During his weigh in's since getting his birth weight back he was hovering along the 2nd centile line and they were happy he was following his own line weighing 3kg two weeks ago.
However on Friday he had gone up to 3.4 kg but went down to the 0.4 centile line

I mentioned possibility of reflux to the HV and she said mention it at your 6 week check (which is on Tuesday) along with him going down to the 0.4 centile line.

These are his symptoms:
*Agitated during and after feeds and looks to be in discomfort/pain

*Pukes most of his milk up both during and after feeding (prob what's causing the lack of weight and frequent feeding) he also projectiles every so often and also when he is sick it comes out his nose too.

*Coughs and chokes during feeding

*Doesn't like being laid on his back and it makes him dribble milk out that's been in his stomach

*Seems very uncomfortable 😣 looks like he making a face of straining to poo but he is not pooing at the time of making this face (he does poo once a day but not with this face expression)

*Plus he smells of sour milk

If relevant he also has yellow/green poo (more green than yellow)
One HV said this is him swallowing too much air while feeding. Another HV said this is normal. Can anyone clarify?

Sorry bit of a long post but thought I'd best put in all the info.

On a separate note do all babies joints click a little? sometimes he clicks, (not frequently).
I've put it down to all babies are flexible and so ligaments are spongy and until they develop more joints are a little clicky

Hi Bendy, they certainly sound like they could be reflux symptoms. Was your son deemed IUGR? It sounds like he possibly was if he was on the 2nd centile having stopped growing at 37 weeks.

If so, did you get a reason - was your placenta sent to a lab etc?

Reflux is more common in IUGR babies, but also IUGR babies can have indeterminate feeding problems, just because.

I really feel for you; it's exhausting worrying about weight and how much they're taking the whole time.

You might find it useful to contact the Child Growth Foundation here or they have a private facebook group you can ask to join, to talk to folk who have been through / are going through similar.

With regard to him spluttering and choking during feeding, it might be worth discussing with your health visitor using a thickener in his milk or at least one of the formulas made for reflux which I think are made to be thicker.

I'm sure you've heard all the basic reflux tips like feed little and often, keep your baby slightly upright if poss (wedge under moses basket mattress? Not sure what is considered safe these days). He may like a sling where he's upright. Sucking on his fingers or a dummy might provide comfort. My refluxy tiny baby also was comforted by a swing, if you can get hold of one.

Good luck; it's exhausting for you. You sound like you're coping brilliantly. BrewCake

Thank you for your reply.

They never mentioned IUGR but he probably is. And after birth they said the placenta was looking absolutely fine.

We had our 6 week check yesterday at the doc did ask why was he small and we said we weren't given any reason. To be fair he was not impressed my notes were not completed well because it didn't state I was induced at 37 weeks and that the reason for the induction was that baby was measuring small.

We have been prescribed baby gaviscon. So giving that a try.

I did forget to ask how long to use it for. I know to use the whole packet but what happens when he has finished the packet? Do I leave it and see how he goes or do I immediately put in his prescription for another packet straight away? Anybody know?

hi again, sorry, only me but at least it'll bump this for other folk!

Grrr re your experience with the notes etc - the medical profession can be so laissez-faire about poor growth/size; it makes me cross!

I hope the baby gaviscon makes a difference for your boy. Yes, I would order another as soon as you are nearing the end of the first, if you think it's making a difference.

I really hope it does. In the meantime, if he falls below the 0.4th, I would push very strongly for a referral to a paediatrician. It's sad that you need to push, but sometimes you do. Best of luck. I hope very much it's a blip.

Thank you. Smile

I weighed him today, he is still gaining weight and he has now risen just above 0.4 but not back to 2 yet.

The HV suggested give him till next Friday and see where he is at then as the gaviscon won't have had chance to kick in enough to help the reflux. Hopfully the "milky volcano" won't erupt quite as much and he can then gain back his 2nd centile line.

Think the gaviscon is kind of working, we are now dealing with hard poo's.
I've given him 10ml of cooled boiled water in between a feed which HV recommended. Hopfully this will help.

Green poos can be a sign of lactose intolerance (are they mucus like).
I hope the Gaviscon works, most people have to go onto ranitidine or omeprazole. Definitely push for a referral. You have to be really pushy, don't let them fob you off!

Before gaviscon they were Yellow and green together and your usual baby poo consistency (like pesto!)

Now we have used gaviscon they are yellow and green together but hard and dry (like pottery clay)

We were told by one HV they were that colour because he was taking in too much air.
Another HV said it's normal.

I have thought about changing his formula but thought I'd try the gaviscon and see if they initially made any difference.

Can't breast feed as he had had tongue tie and also I don't produce enough to pump (a perthetic 5ml a day Confused )

Gaviscon can definitely constipate them.
You can get colief drops to put in the formula that breaks down the lactose. There are also lactose free formulas you can buy.
