domperidone stopped

Am at the end of my tether with our poor Lo and his reflux. He was 7wks prem and was possetty and very colicky as a newborn. Gp at Xmas prescribed Infant Gaviscon which only really helped for a while then I was told things would improve on weaning. It took 3 attempts to wean due to vomits and screaming obviously with the acid and we finally got going with around April time at 8 months old. Long story but things just got more and more worse with vomiting up meals or milk and gagging on any kind of lumps. He went from nearly the 50th centile in January right down to the 2nd where he has been since June. Pediatrician and dietician put him on domperidone and ranitidine just over a month ago and diseases have been increased then put upto max for his weight 10 days ago due to reflux cough returning and vomiting. Two days after domperidone was increased to max, he started vomiting every day again and it was quite clear it was undigested solids from upto 3 hours prior as we still even at 13 months have to keep at least 2 hours between any kind of feeding. Back to the gp today and she suggested stopping the domperidone.

Has anyone else experienced similar by over a year old although prem? It is still hit and miss with finger food as to whether he will gag and vomit or not even on softly cooked to death veg. I am really getting worried now that by still being on baby pureed food at 13 months old will really hinder his eating development. The doc was surprised that he no longer possets especially now crawling as she said that's what normally happens with reflux so if course now I am worrying it us something more sinister than reflux.

Of course on a bad day also, he wakes in the night needing a top up milk feed or cough medicine which after having our sleep back since Xmas and returning to work is really taking its toll!

Sounds like something isn't right and I'd ask for an immediate second opinion from another paed with an interest in gastro - if you're Essex/Herts I can recommend one.

I'm at a loss as to how stopping domperidone is likely to help things, and for sure he shouldn't be on regular cough medicine! What's causing the cough is the question! Has your paed talked about other drugs, and whether he has simple/silent reflux or CMPI?

There's a lot of unanswered questions there which I think you need to get to the bottom of. FWIW my 8 mth (prem) twins are on domperidone every feed plus gaviscon, and one is on neocate, omeprazole and ketotifen and the other is on enfamil AR. We're weaning with huge caution and have been told to avoid egg, soya, gluten, milk, cheese, wheat and some seeded fruit. They were both on ranitidine but have slowly come off that without any ill effect.

I hope you get some answers soon.

Just to add, I would ONLY take notice of the clinical opinion of someone for whom this is their specialist area, and that's unlikely to be your GP, whose suggestion to stop domperidone sounds frankly, foolish.
