Help needed with son’s endoscopy results

My 13 year old ds recently had an endoscopy to rule out eosinophillic oesophagitis and check for signs of reflux.

He has regular pain, brings up parts of his meals and also has intermittent swallowing issues. He has also had longstanding issues with constipation. Although this is better now he’s a bit older and he hasn’t had a serious issue or impaction for a few years.

A barium swallow showed no swallowing issues (swallowing issues were put down to his hypermobility spectrum disorder) but did discover that he has a malrotated gut. We are back to see the surgeons this afternoon to find out whether or not he will have to have that surgically corrected.

Endoscopy showed no sign of reflux or eosinophillic oesophagitis.

Gastric body and antrial biopsies showed chronic, non-specific inflammation, although it says this was minimal.

His total IGE was elevated 112 (normal range 0-100)
Negatgive TTG
IGE to common food mix 0.47 (normal range 0.01-0.35
IGE to wheat, soya, milk, peanut, egg and fish all normal
All the other bloods, U&Es, FBC, LF, CRP etc were within normal range

A quick google suggests Helicobacter could be a potential cause for the inflammation on the biopsies. His grandmother had a chronic case and her husband and siblings all had to be treated as they had it too.

I am confused about the IGE results though, as if he’s not allergic to any of those food groups, why is his IGE raised?

Report summary says the endoscopy was normal and there was no sign of inflammation that would explain his symptoms. Gastro said they suspect his symptoms are secondary to his Hypermobility Spectrum Disorder, which is difficult to manage, but should be managed ‘expectantly’. She is going to have us back to clinic for a review.

The thing is, I know from experience that our family’s results to tests tend to be atypical and I don’t want to risk being fobbed off, when the results did show some things aren’t completely normal, even if they are not drastically bad. So I would like to understand the implications of his results, if possible, a bit better before we go back to clinic.

Some good news. The surgeon we saw today confirmed he doesn’t have to have the surgery and that he was tested for Helicobacter and it was negative.

Just spotted this.. Are you part of the parents of older refluxers Facebook group and the EGID and associated conditions one? They have a wealth of experience with this type of stuff and can often make good suggestions. Did the consultant help you come up with a management plan? The symptoms alone presumably need managing to reduce the inflammation. (My DS1 has lymphocytic oesophagitis.)

And Moose, I remember us being on a chat together about gastro issues some years ago. We are still dealing with ours too, although my eldest has been diagnosed with lymphocytic oesophagitis (similar to but rarer than eoe apparently).
