Is renewing DLA straightforward if AC‘s surname has now changed?

Just had the form to reapply for DD‘s DLA and it has her birth name on it. Does anyone know if there is a simple way to sort this out which doesn’t involve the DWP cocking it all up?

Best too speak them and Ask advice too get it right, would be different areas I would think, such as Kinship, SGO's. We are on carer's element which is in child's name are now in the process , moving and applying for Full DLA , DLA sent us new application forms all Birth Certificate's and supporting information will be sent too them along with supporting evidence , ETC . But still best too speak too them before you proceed will be given new Ref No and so on. As you proceed and need too ask further advice, all will or should be on record. Hope this helps

My son was in receipt of DLA when he arrived, we had to do something to transfer the payment from the FC to me but I can't remember what ! It came up for renewal two years post adoption. His adoption was heavily referenced in the application but I don't recall having to send in adoption certs. Whatever I did, it was all very smooth, no problems at all.

The form does ask if the child has been known by another name

Ok, thanks, I have missed that obvious bit. She didn’t have DLA when she came as FC said they couldn’t claim it for some reason. I filled in the form and got it a few months after she arrived (annoyingly not back-dated to her arrival though).
